**Responsibilities**

The Infectious Diseases Clinical Pharmacist Specialist manages the Hospital Antibiotic Stewardship Program (ASP) by providing collaborative pharmaceutical care to Hospital patients through the ASP with the support of the Infectious Disease physician(s), faculty and house staff physicians, pharmacists and clinical microbiology staff. As required, the Infectious Diseases Clinical Pharmacist Specialist provides comprehensive pharmaceutical care through integrated drug distribution, patient counseling and teaching. Clinical Pharmacists fill orders for medications, monitor patients' drug therapies and provide drug information. Clinical Pharmacists supervises and directs supportive staff. In addition, the Clinical Pharmacist provides pharmaceutical care services to include advising physicians on matters pertaining to drug usage and control within assigned hospital locations; participate in the development and delivery of continuing education programs for Pharmacy, Patient Care Services, Medical staff and students.

**Qualifications**

**POSITION REQUIREMENTS**

Education/Qualification

+ Must Possess a Pharm.D. degree

+ Licensed (or eligible for licensure) to practice in the District of Columbia

+ Completed a Pharmacy Practice Residency (or equivalent) with experience in Infectious diseases.

+ Completed a specialized clinical residency/fellowship in infectious diseases (preferred)

+ BPS Board Certified Infectious Diseases Pharmacist (preferred)

+ Effective communication skills, both verbal and written

+ Ability to maintain confidentiality

+ Ability to adapt to multiple and changing priorities

+ Knowledgeable of and complies with laws, rules and regulations that govern the practice of pharmacy

+ Pyxis experience, with knowledge of Pyxis reports (i.e. optimization, load/unload, nursing narcotics access, etc)

+ Ability to use Cerner, Microsoft Excel, PowerPoint

**RESPONSIBILITIES**

The major job responsibilities of this position are: pharmaceutical care to Hospital patients receiving antimicrobial therapy, clinical consultations, cost effective care analysis, teaching, and Pharmacy representation on Hospital committees.

The following activities are components of the position to assist in meeting the major job responsibilities:

+ Provide cost-effective pharmaceutical care to patients receiving select antimicrobial therapy.

+ Participate in selection of patients receiving antimicrobials and monitor these patients on a daily basis.

+ Discuss medication order changes/clarifications with the Infectious Diseases physician(s) or the prescriber, document any changes in patient and pharmacy records, and inform others of medication order changes.

+ Monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions. Switch patients from IV to PO therapy as quickly as feasible.

+ Provide pharmacokinetic consultations as required.

+ Detect, monitor, document and report adverse drug reactions (ADRs) and medication errors (MEs)

+ Promote the use of the formulary by converting non-formulary orders to formulary when possible, but coordinates procurement of non-formulary drugs when necessary.

+ Maintain and update patient profiles with demographics, diagnosis, allergies, and current medications.

+ Facilitate discharge for home care patients

+ Anticipate discharge date and medications.

+ Facilitate discharge planning and smooth transition to alternate site of care or the outpatient pharmacy.

+ Provide discharge medication counseling to patients.

+ Work collaboratively with the Infectious Diseases physicians or faculty service to ensure appropriate practice in the ambulatory setting.

+ Provide limited operational support to the Pharmacy

+ Review select patients' antimicrobial therapy for accuracy and appropriateness

+ Communicate with Pharmacy and nursing staff to avoid waste in preparation of antimicrobials

+ Promote and demonstrate rational, cost-effective drug therapy

+ Identify and implement drug therapy where positive pharmacoeconomic impact could be achieved. The ultimate goal is to develop and assure cost effective use of antimicrobials, including antibiotics, antifungals, and antivirals.

+ Develop and conduct target drug programs and usage evaluations as needed, and report results to the Pharmacy and Therapeutics (P&T) Committee.

+ Participate on the Antimicrobial Subcommittee (or similar entity) of the P&T Committee.

+ Document clinical consultations and cost avoidance activities as part of the justification for the position/service.

+ Monitor the use of expensive medications to assure there is use consistent with approved criteria.

+ Anticipate changes in drug therapy, and report the potential impact it will represent to the overall Pharmacy and Hospital drug budget.

+ Facilitate investigational drug studies

+ Take a role in the teaching programs of the Department of Pharmacy Services, as they relate to infectious diseases

+ Precept Pharm.D. students

+ Provide staff development support to pharmacists, nurses and physicians

+ Provide presentations, publications, and other informational activities on infectious diseases.

+ Participate in appropriate departmental quality improvement initiatives
