Job Summary
Provides personal health care and related services to clients in the home. Assists in providing a safe and hygienic environment, works cooperatively with the patient and family, and shares observations and problems with supervisor. Works under the direct supervision of a registered professional nurse or appropriate licensed therapist.

Essential Functions


  • Performs personal care and health related household tasks as directed which may include: backrubs, bed baths, showers, tub baths and skin care.
  • Positions, ambulates and transfers patients according to the plan of care and as individually instructed. Tasks may involve turning and positioning patients in bed, assisting to get patient in/out of a chair or wheelchair, walking with the client and assisting with walker or cane.
  • Assists patients with exercises such as range of motion and ambulation taught by appropriate health professionals. Encourages patient to become independent and progressively more physically active.
  • Ensures patient receives prescribed diet, proper and prescribed nutrition and food/fluid intake and assists with planning, preparing, serving meals and assisting with feeding as needed.
  • Performs vital signs as directed including temperature (oral, rectal or axillary), blood pressure, pulse and respirations. Weighs patient as instructed. Assists with medications as instructed or assist the patient with self-administration (open medicine bottle or provide glass of water).
  • Performs and documents treatments as directed in the plan of care. Treatments may include monitoring and measuring intake and output, weighing patient and recording, reinforcing dressings and applying abdominal binders.
  • Observes isolation precautions and follows policies and procedures. May perform special tasks only if instructed to from the registered nurse and after a successful return demonstration: assisting patient/family to change colostomy bag (no irrigations), urinary catheter care, ice collar or cap care, soaks and compresses—non-sterile (not hot), treat abrasion or superficial wounds—only those not requiring medications or sterile dressings.
  • Teaches safety, personal hygiene, household skills, caregiver skills and measures to prevent the spread of disease, i.e. handwashing. May perform light housekeeping in the immediate patient environment and may launder patient linens or prepare light meals.
  • Documents all care rendered, patient observations, problems and interventions, and communicates pertinent information to the nurse/therapist. Reports any changes or deviations in the patient schedule to the registered nurse or licensed therapist.

Physical Requirements
Fast-paced environment with numerous interruptions. Sits, stands, bends, lifts seven to eight hours per day. Rapid work, ability to perform tasks involving physical activity which may include heavy lifting and extensive bending and standing. Ability to travel extensively during all seasons into assigned patient homes and work in various home conditions and temperatures. Lifting of patients, equipment or supplies up to 50 pounds frequently and up to 100 pounds with assistance.

Education, Experience and Certifications
Completion of ninth grade education required. High School Diploma preferred. At least one year experience as a nurse's aide in a hospital or nursing home, or successful completion of a certified home health aide program. BLS HCP from AHA required. Valid driver's license and auto insurance required in State of residence.
