This Health Education position supports the Comprehensive Health Care Center, a teaching site and is being submitted to PCC after the unit's transition into the Office of Community & Population Health. The Health Educator provides assessment of individual patient and community needs for health education at the Montefiore the Montefiore Medical Group Ambulatory Network. This position participates in planning and implementation of effective health education strategies and interventions. Health Educators are a part of the care team and play a critical role in the Asthma, CVD, Tobacco and Diabetes DSRIP projects. The Health Educator provides individual and group counseling/ education sessions for MMG patients. This position will also provide general health education services for patients that are referred to by the Primary Care Providers across all of MMG. Health Educators are also trained lifestyle coaches implementing the CDC Diabetes Prevention Program intervention for pre-diabetic populations at Montefiore.  


This position is accountable for screening and assessing the educational needs of our patient population and their families in turn developing, organizing, implementing and evaluating effective methods to meet those needs. It also involves developing and organizing materials for staff and coordinating all community outreach and educational programming, including lectures, classes and workshops.


Masters degree in Health Related field preferred.

Bachelor Degree required. 

CHES Certification required.

Bilingual Spanish required, previous experience preferred.
