The Director, Home Health directs the operations, programs, and services of Home Health or Hospice.  Establishes, implements and evaluates the goals and objectives of the facility’s services to ensure they meet quality standards and support the organization’s philosophies.



  1. Provides direction to managers, staff, and other personnel in the understanding and achievement of Home Health’s vision, goals, and objectives through meetings, oversight, and precept mentoring. Ensures the professional development of these individuals by providing education and training on new procedures and processes and/or providing opportunities for external classes and seminars.
  2. Establishes interdisciplinary teams to plan for and address issues with changing patient care delivery system and services. Identifies when a team needs additional resources and collaborate with managers to assign more team members. Provides guidance and follow-up with the team to ensure that any changes are implemented as planned. 
  3. Participates in growth strategy sessions for the purpose of identifying new services, developing a business plan and implementing approved programs.  Provides statistics from Presence Health entities and referral and gathers and provides feedback from stakeholders and professional advisory group members to assist in the decision-making process.
  4. Consults with officer and leadership on organizational goals and facilitates communication between leadership and clinical staff to promote and achieve these goals.
  5. Develops and implements standards of practice within areas of responsibility that enhance quality of services provided and ensures compliance with regulatory agencies by remaining current on these regulations and informing others of updates and/or changes needed.
  6. Oversees all direct and indirect patient/client services provided by clinical ministry staff to ensure that care is delivered in a safe, efficient manner.  Identifies and resolves any issues through follow-up with appropriate individual(s).
  7. Collaborates with clinical managers in overseeing clinical teams and in planning for area(s) of responsibility. Guides and counsels coordinators and clinical supervisors to assist them in improving all aspects of home care or hospice services provided through the staff.
  8. Serves as a steward of organizational resources by ensuring proper correlation between expenses and operational volumes. Monitors and interprets key operational indicators, identifies variances, and makes appropriate staffing and expense control actions to meet these stewardship objectives.
  9. Monitors reports regarding patient/client clinical records to ensure the records are complete and that documentation is compliant with clinical, regulatory and billing standards.

Education and/or Experience                                                             

  • Associate's degree in nursing or a health related field with a minimum of seven years clinical experience, including five years of progressive management experience; or Bachelor’s degree in nursing or health related field with a minimum of five years clinical experience, including three years of progressive management experience.
  • Demonstrated strong leadership skills and excellent project management and organizational skills.
  • Ability to respond quickly in an emergency and knowledge of state and federal regulations and standards.
    • Knowledge of Microsoft Word and Excel required.


    Certificates, Licenses, Registrations

    • Registered Nurse with a current Illinois License required.
    • CPR certification required.
    • Current Illinois driver’s license, access to a vehicle and proof of auto insurance required.
