CLINIC: Centura Health Physician Group (CHPG) St Anthony North Family Medicine – Westminster, CO

DETAILS: This is a Per Requested Need (PRN) or “as needed” position. There are not a guaranteed number of hours offered each week.


Occasional travel to other sites to meet the needs of the clinic may be required.

Position Summary

Diabetic Educator will work cooperatively with the multidisciplinary team on curriculum development and teaching materials for educational services to diabetic patients. He/she will explore all options to procure testing monitors, materials and community resources for the un/under insured. Acts as the coordinator and lead professional instructor of the AADE certified diabetes program following policies, protocols and other required standards to apply and maintain AADE certification. Responsible for teaching diabetes education care skills to patients with Type 1 and Type 2 diabetes in class and individual sessions. Responsible for maintaining optimal program development, polices, marketing and data outcomes. The educator will collaborate with a multidisciplinary team to provide quality care for patients enrolled in the diabetes program. The Diabetic Educator will acts as a leader, innovator and change agent in the pursuit of improved quality of patient care.

Minimum Education Requirements

  • Registered Dietician or Nurse

  • Certified Diabetic Educator or equivalent training/education

Minimum Experience Requirements

  • Three years’ experience in patient education

  • One year experience in diabetic education

Position Duties (essential functions denoted with an * )

  • Develops and provides individualized diabetes self-management/training by incorporating evidence based research, principles of adult learning, AADE self-care behavior theory to empower the patient, family with diabetes to manage the chronic disease.*

  • Follows Cardiac Rehabilitation Department policies as well as the AADE scope and standards of practice.*

  • Maintains certification requirements of the program by following AADE policies and submitting annual outcomes assessment.*

  • Leads diabetes advisory group meetings comprised of a multidisciplinary team that oversees quality of the program.

  • Documents assessment, ongoing status, interventions, patient response and outcomes in an accurate timely manner.

  • Maintains thorough and confidential maintenance of patient charts and records. *

  • Monitors referrals and patient enrollment process with multidisciplinary staff from centralized scheduling and health benefits staff. Cooperates with insurance pre-authorization and documentation requirements for compensation.*

  • Designs curriculum, class schedule diabetes support group events, and marketing for the program.*

  • Explore all options to procure testing monitors, materials, and community resources for the un/under insured.*

Physical Requirements

  • Light Work - exert/lift up to 20 lbs. force occasionally, and/or up to 10 lbs. frequently)
