The Coordinator, Health Information Management and Credentialing manages the day-to-day operation of the Medical Records Department.   



  1. Performs in-house and Discharge Chart Analysis for completeness of record. Processes incomplete medical record information and forwards the information to the physician’s office and to appropriate facility department for completion.


  2. Manages facility Master Patient Index File. Performs ICD9-CM Coding for patient diagnosis with assistance from the department manager.


  3. Processes correspondence requests such as subpoenas, requests from attorneys, Risk Management, residents, families, and insurance companies.


  4. Serves as an information source for clinical departments and facility staff.


  5. Prepares credentialing application documents and forwards to the requesting physician. Analyzes returned credentialing application for completeness. Forwards all completed credentialing documents for approval to the Medical Director and Administrator.

    Education and/or Experience                                                              

    High school diploma or GED required. Minimum one year of experience in a health care setting required. Knowledge of Medical terminology required.


    Previous experience in a Health Information Management Department preferred.


    Computer Skills                                                          

    Knowledge of Microsoft Word and Excel required.
