SUMMARY

The Intake Nurse is a Registered Nurse (RN) that practices within the scope of the Illinois Nurse Practice Act to ensure the delivery of comprehensive, high quality, individualized nursing care, based upon knowledge and experience the nursing process and leadership skills. Receives and processes initial patient referral information for Home Health Services (HHS) and Home Medical Equipment (HME), coordinating with physician offices, emergency department, other healthcare facilities, and other ancillary areas.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Serves as a centralized intake of referrals. Processes initial orders for referred patients to HHS; reviews intake information for completeness; inputs data into computer system; and contacts potential clients’ physicians for patient information as needed.

     

  2. Serves as a resource to nurse liaisons, physicians and discharge planners regarding HHS, Medicare, private insurance and public aid benefits. Communicates agency services and philosophy to all referral sources all callers seeking home care information.

     

  3. Maintains knowledge of Medicare and Medicaid guidelines regarding eligibility requirements, coverage of services and Conditions of Participation.

     

  4. Coordinates requests for multiple home health and home medical equipment services.

     

  5. Facilitates nurse to nurse reports if required, adding disciplines and physician orders when indicated.

    Education and/or Experience                                                              

    Graduate of an accredited Nursing Program required. Minimum three years of clinical experience required.

     

    Three years of experience in home health preferred.

     

    Computer Skills                                                          

    Basic computer skills required.

     

    Certificates, Licenses, Registrations

    Registered Nurse with current Illinois License required. Valid driver’s license, current automobile liability insurance and access to an automobile during work required.
