Position Summary



Responsible for supporting communities as part of Centura Health’s Community Health Advocate (CHA) team. CHA’s are public health workers and trusted members of the community. They serve as a link between patients and social service agencies in the community to help members gain access to available health programs. Provide consumers with information and assistance to access the health care and enrolls our community members in public insurance programs. Utilize knowledge and skills in community health improvement and care management to support the Community Health Needs Assessment Implementation plan.



Minimum Education Requirements




  • High school Diploma or GED

  • Bachelor’s degree strongly preferred

  • Previous Community Health Worker training preferred



Minimum Experience Requirements




  • Two years of experience working in a Community Health Worker (CHW) role or experience with social services, support services, public health or related health outreach, preferred

  • Experience interviewing and collecting confidential data, and maintaining records

  • Experience in completing standard forms; previous experience with Connect for Health Colorado enrollment a plus

  • Written and oral fluency in English

  • Written and oral fluency in other languages spoken in the assigned geographic region (particularly Spanish)

  • Good technical computer skills using Excel and other software programs

  • Good presentation skills

  • Excellent interpersonal skills

  • Ability and willingness to provide emotional support, encouragement and motivation to clients to fully participate in service programs

  • Willingness to distinguish clients with severe social, emotional needs and refer them to the appropriate professionals and resources

  • Ability to demonstrate cultural awareness and humility



Position Duties (* denotes an essential functions of the position)




  • Conduct intake interviews with clients. *

  • Work with hospital team staff to identify difficult-to-reach clients and provide strategies for reaching them. *

  • Make follow-up phone calls and home visits or visits to other appropriate settings (community based-organizations) to explain and enroll individuals in appropriate services such as health insurance. *

  • Schedule appointments for clients and ensure the attendance at appointments. *

  • Collaborates with team members with data maintenance and report preparation. *

  • Assist clients in addressing barriers to care such as transportation, language, family care, etc. *

  • Follow-up with clients by letter, phone call, or home visit on missed appointments or referral. *

  • Establish relationships with clients and provide general support and encouragement for clients. *

  • Provide general support for hospital team staff. *

  • Works collaboratively with community-based service providers. *

  • Participates in personal, professional and program development by attending department in-services, meetings and/or trainings. *

  • Assists in training new staff; acts as a resource for co-workers. *

  • Actively participates in quality improvement initiatives. *

  • Adapts to rapidly changing technologies, policies, regulations, and staffing through priority setting and flexibility. *

  • Demonstrates effective time management and is proactive in prioritizing workload and meeting program expectations. *
