POSITION SUMMARY



This position is a combined position of both Health Unit Coordinator and Certified Nurse Assistant. This position is an upgraded position that requires the position holder to be competent in both the HUC and the C.N.A. job skills.  These job skills and performance may be on separate days, or the position holder may be asked to perform a combination of the skills during the same shift day.  These job skills are performed in collaboration with the healthcare team.



The HUC works in collaboration with the healthcare team, the HUC coordinates and prioritizes patient, physician and staff needs.   The key essential job functions of this role include customer focus, coordination of provider-entered orders, communication, patient experience activities and time management.



The CNA works under the direction of the RN.  The scope of responsibilities include:  data collection related to patient care, administering treatments and procedures, communicating patient responses of the care provided to the RN.  Under no circumstances may the CNA delegate tasks or supervise other members of the health team.  The CNA will assume personal accountability for organizational mission, core, and SHARE values.



EDUCATION REQUIREMENTS




  • High school diploma or GED preferred



WORK EXPERIENCE REQUIREMENTS




  • Medical Terminology, clerical skills, related experience preferred

  • Proficient in basic computer skills

  • Health Unit Coordinator with at least one year experience preferred.

  • Successful passing of written pre-assessment required.  Completion of St. Mary-Corwin HUC orientation.



LICENSE/CERTIFICATIONS




  • Nurse Aid certification from Colorado State Board

  • Current BLS certification required from American Heart Association

  • Uses resources in a cost effective manner including helping to meet unit productivity standards.

  • Infection control standards are maintained through proper handwashing, utilization of standard precautions and following isolation procedures



Documentation (10%)




  • Complete established documentation tools timely, accurately, and thoroughly



Clinical Practice (60%)



These functions are the functions during the duty as the HUC




  • CUSTOMER FOCUS:

    • Welcomes and greets patients/visitors. Directs/escorts customer to desired location

    • Triages patient call lights in a timely manner.

    • Communicates with patient, family members, visitors, physicians and associates in a competent, caring manner.



  • COORDINATION OF PROVIDER-ENTERED ORDERS:

    • Monitors the electronic record for new provider-entered orders, coordinating the completion of the orders with nursing staff and other departments as appropriate.

    • Competently utilizes computer technology to access and exchange data (ie lab results, interdepartmental communications)

    • Maintains non-EMR patient records as needed to ensure availability of accurate patient information.

    • Ability to use critical thinking skills competently in order to handle multiple tasks and fluctuating volumes.

    • Assists in the coordination of physician consults.



  • COMMUNICATION:

    • Proactively identifies and communicates clearly with all members of the team to ensure that everyone remains informed of tests, procedures, messages, new orders and needs of patients.

    • Effectively communicates with medical staff, nursing and other associates to ensure that timely care is delivered



  • PATIENT EXPERIENCE ACTIVITIES:

    • May  on Patient Experience Committee.



  • TIME MANAGEMENT:

    • Independently prioritizes tasks and maintains an uncluttered work environment.

    • Maintains adequate supplies. (Office and Medical)

    • Input of daily and special charges when required.  Maintains/updates unit logs  in timely manner.

    • Filing, logging, copying, scanning, reporting and other clerical duties as required.

    • Coordinates services from maintenance and other service departments as needed

    • Orients/precepts new staff as requested





These functions are the functions during the duty as the C.N.A.:




  • SAFETY:

    • Protects patient by following fall risk precautions.

    • Understand and practices safety for patients that are placed in all types of restraints.

    • Assures safekeeping of patient’s belongings by following established processes.



  • ADL’S:

    • Meets the personal care need through:

      • daily bath

      • hair grooming

      • oral care

      • nail care

      • male and female perineal care

      • linen changes








  • Provides toileting assistance through:

    • bowel/bladder programs                     

    • bedpan and urinal usage

    • bedside commodes                                                    

    • urinary drainage and ostomy devices

    • ambulation to bathroom           

    • enemas as delegated by the RN

    • Collects noninvasive bowel and bladder specimens through delegation of the RN






  • NUTRITION:

    • Helps the patient meet nutritional needs:

    • Sets up meal tray and nourishments

    • Assist in feeding and/or providing adaptive equipment

    • Monitors and records correct diet and % of meal intake

    • Monitors and records accurate I&O’s

    • Obtains and records accurate patient weights



  • MOBILITY:

    • Ambulates patient per order with appropriate use of assistive    

    • devices, including gait belts

    • Performs ROM and exercises at the direction of the RN

    • Arranges and/or provides safe transfers and transportation using appropriate equipment

    • Maintains working knowledge of bed applications



  • PATIENT CONDITION/STATUS:

    • Observes and reports (to RN) change in patient conditions or behavior promptly, or any concerns regarding patient/family

    • Measures and records vital signs and promptly reports abnormal findings and/or changes to RN

    • Provides care appropriate to age group or special developmental needs of patients

    • Provides appropriate end of life and post-mortem care
