Description
* Monitors patient laboratory indices and makes appropriate dosing recommendations as necessary.
* Participates in pharmacy dosing services which includes but is not limited to pharmacokinetic dosing, anticoagulation, parenteral nutrition and other clinical dosing as directed by organizational protocol.
* Identifies, recommends, resolves, and prevents medication - related problems through direct pharmacist intervention with physicians, including documentation of the intervention.
* Provides initial clinical screening and monitors drug therapy regimens for contraindications, therapeutic duplication, drug - drug interactions, drug-food interactions, allergies, appropriateness of drug / dose, renal dosing adjustment, and IV compatibility. Ensures patient allergies are documented and verified against each medication ordered. Detects, assesses, and reports suspected adverse drug reactions.
* Participates in the Code Blue Cardio / Pulmonary or other Response Team as directed by site specific dept. procedures.
* Interprets medication orders accurately, including age appropriate dosing. Enters or verifies orders in the pharmacy computer system.
* Participates and adheres to system/site antimicrobial stewardship strategies.

The Clinical Pharmacist I is responsible for overseeing the preparation & dispensing of medication, medication order computer entry, the monitoring of patient medication therapy, and acting as a resource for the provision of medication information for patients, physicians, nurses, and other allied health professionals. The pharmacist identifies, recommends, resolves, and prevents medication - related problems through the intervention with physicians. Pharmacists supervise and direct pharmacy support personnel.
