Duties and Responsibilities: Directs and coordinates the delivery of all pharmacy services on the assigned shift, without direct supervision within the limitation of department policies and procedures. Accepts and executes #charge pharmacist# responsibility as assigned. Prepare, compound, dispense and deliver medications to patients in accordance with the Board of Registration in Pharmacy, the Department of Public Health, State Law, Federal Law, and professional pharmacy practice standards. Evaluates medication orders and patient medical records for medication related problems. Documents, and resolves medication related problems. Communicates professionally with Physicians, Nurses and other healthcare professionals regarding medication related issues. # Performs all other tasks promptly as assigned.# Documents competency by routinely documenting clinical pharmacy interventions. Serve as a drug information resource to physicians, nurses, patients and other members of the healthcare team. Maintains skill, knowledge and competency through department initiated trainings, attendance at journal clubs, attendance at external trainings, routinely reviewing medical literature. Performs medication counseling and medication history taking when required, expertly performs basic patient care services such as blood glucose testing, blood pressure monitoring, peak flow meters, BLS/ACLS (if required by care setting). Participate in department meetings, and pharmacist practice council. Orients and trains pharmacy interns, residents, technician trainees, other students and newly hired personnel. Manage support staff and verify the accuracy of their work. Work with the Pharmacy Technician Supervisor to accomplish departmental tasks and realize operational efficiencies.
