Description
The staffing for this position will be primarily in cardiology with rotation through other inpatient areas. Prior cardiology experience preferred.

Dispenses medications, monitors patients' drug therapies, and provides consultation to medical and nursing staff.

* Evaluates, prepares and dispenses medication orders.
* Oversees pharmacy support personnel activities related to the accurate compounding and dispensing of pharmaceuticals and redistributes resources as needed to support workload.
* Ensures safe, appropriate, and cost effective medication therapies for patients. Monitors medication therapy by assessment of therapeutic appropriateness and duplication of the regimen, appropriateness of the route and method of administration, and patient compliance with the regimen.
* Reconciles medication lists upon transitions of care.
* Extracts and interprets information in patient charts.
* Detects and reports suspected adverse drug events, medication errors and near misses.
* Sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, Pharmacy and Therapeutics Committee approved drug use criteria; and promoting rational drug therapy selection.
* Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost effective therapeutic alternatives to medical staff.
* Manages medication therapy where defined by protocol.
* Provides drug information to the professional staff.
* Assists in discharge planning. Counsels patient/families concerning proper drug administration, potential drug side effects and precautions, and discharge medication orders. Helps locate medication supplies not generally available.
* Documents all clinical activities and interventions.
* Participates in the quality improvement and medication use review activities of the department.
* Participates in the development and presentation of orientation, education, and training programs for the pharmacy, medical, nursing and other staff members.
* Oversees and directs Pharmacy support personnel. Verifies the daily activities of assigned staff. Provides input into support personnel performance reviews.
* Ensures readiness for regulatory and accreditation surveys.
* Precepts new hires and pharmacy students
* Performs other duties as required or assigned.

Minimum Qualifications:

* Graduate of an ACPE-accredited School of Pharmacy with a Bachelor's of Science in Pharmacy
* Doctor of Pharmacy (Pharm D) degree preferred.
* Completion of a post-graduate residency and/or board certified preferred.
* Must be able to work independently with minimal direct supervision, to set priorities, make critical decisions, respond quickly to emergency requests, handle frequent interruptions, and adapt to changes in workload.
* Maintains customer-service focus to achieve excellent working relationships.
* Strong written and verbal communication skills are essential.
* Must be currently licensed to practice in the state of NH, and be willing to seek license reciprocity in multiple states if the position requires it.

Required Licensure/Certification Skills:

* Must have a current, unrestricted pharmacist licensed to practice in the state of NH.
* If practicing at a VT location, must have a current, unrestricted pharmacist license to practice in VT.
