Clinical Pharmacist - Refill Authorization

POSITION DESCRIPTION

The Clinical Pharmacist for refill authorization will support the operations of a Prescription Renewal Authorization Center, utilizing a Collaborative Drug Therapy Agreement with physicians to perform refill authorizations, therapeutic interchange, and other duties according to established policies and procedures. The refill pharmacist will be a resource for Polyclinic providers and staff with inquiries regarding medications and drug interactions and must be able to work effectively as a highly dependable member of an inter-professional team. Effective communication with patients, physicians, staff members, and outside pharmacies is highly sought after. We are seeking an individual who has a highly self-motivated work ethic and desire to continually improve workflow and patient experiences.

EDUCATION

* Graduation from an accredited school of pharmacy with a B.S. or Doctorate in Pharmacy Practice
* Completion of a PGY1 Residency with a strong ambulatory care component preferred

LICENSURE

* Current WA State Pharmacist License in good standing

EXPERIENCE

* 2 years' experience directly related to this position (examples include: 2-3 years spent in a fast-paced community pharmacy/clinic, previous refill authorization experience). Completion of PGY1 residency satisfies this requirement
* Experience working with electronic medical record systems, preferably EPIC

LOCATION

* This is a fully remote-work position after successful training and completion of competencies. Training is estimated to take 6 months and will be completed at The Polyclinic Broadway location.
