**Role Summary:**

The Clinical Pharmacist will play a critical role in executing the market/segment aim of supporting doctor-patient-community engagement by enhancing the quality and affordability of care. The Clinical Pharmacist will successfully service, monitor and grow strategic relationships with providers by working side-by-side with health care providers to help improve the quality, value and experience of care for their patients, to make health care better for everyone. This individual will focus on supporting providers and their office staff through practice transformation, performance management support, and clinical program integration.

The Clinical Pharmacist will serve as the liaison to physicians and to health systems. This role will work closely with several departments internally, including: market leadership, Pharmacy, local HEDIS & Stars team, Network Operations, Health Services, Sales, and the Data and Analytics team.

**Responsibilities** **:**

+ Supports physician groups by analyzing performance reports and data to guide decision-making, process, and program implementation

+ Monitors and implements educational and process interventions based on practice-level data, trends and identified opportunities. Inclusive of, but not limited to:

+ Advising primary care practices, physicians, nurses and other clinical staff to assist them on their conversion to value-based care

+ Dissemination and support of quality performance data

+ Testing and refining of workflows that support sustainable transformation to provider practices

+ Guiding practices as they expand interventions to additional patient populations

+ Responsible for utilizing available data to implement pharmacy initiatives, such as HEDIS/Star measures

+ Prepares and delivers presentation material at provider meetings within the designated territory.

+ Supports clinical programs aimed at increasing quality while decreasing overall cost of care within high risk populations.

+ Utilizes Pharmacy Benefit Manager resources and maintains comprehensive knowledge base on all heath plan pharmacy benefits

+ Participates in patient rounds as a member of the multidisciplinary care team where applicable.

+ Responsible for understanding and enhancing clinical programs, identifying practice opportunities and integrating programs and resources into the practice workflows with the intent to optimize the patient experience, improve quality of care, and positively impact resource utilization

+ Travels throughout designated territory to meet with physician practices and health systems in support of clinical practice

+ Serves as a drug information resource to internal/external customers

+ Supports the market through participation as Part D/medication SME in provider-facing meetings such as Joint operating committees, care coordination, and onsite CHTs

+ Other Duties as assigned

**Qualifications:**

+ Bachelor's degree in Pharmacy required; PharmD preferred

+ Licensed Pharmacist

+ 5+ years of clinical Pharmacy experience required; experience in managed care or ambulatory care preferred

+ Ability to work in a fast paced and ambiguous environment

+ Strong learning agility and ability to apply concepts quickly

+ Strong problem solving and decision making skills

+ Excellent communication skills, both verbal and written

+ Familiar with CMS Medicare regulations concerning STARs and HEDIS measures

+ Ability to develop innovative solutions to complex healthcare issues

+ Proficient technical skills utilizing MS Office applications (Word, Excel, Outlook, PowerPoint)

+ Ability to travel up to 25% of the time within the assigned territory
