About the Job

The Clinical Pharmacy Specialist - Population Health works collaboratively with other health care providers and disciplines within the Clinical Excellence Division to improve patient outcomes and minimize total cost of care by promoting high-value, evidence-based pharmacotherapy. This is achieved through supporting Beth Israel Lahey Health Performance Network (BILHPN) clinicians through collaboration on the development and implementation of tools, programs and best practices to optimize medication outcomes and value-based contract performance. The scope and nature of patient care services are focused on optimizing patients' therapeutic outcomes, and includes active involvement in interdisciplinary care activities, pharmacotherapy consultation, medication therapy monitoring, provision of drug information, education to both health care providers and patients, and guideline development and implementation. Additionally, the Clinical Pharmacy Specialist supports the safe, effective and efficient use of medications through Pharmacy and nical Pharmacy Specialist supports the safe, effective and efficient use of medications through Pharmacy and Therapeutic Committee involvement and the development of prescribing guidelines, provider educational programs and network-wide communications.

Primary Responsibilities:

1. Collaborate with other members of the health care team to develop prescribing initiatives to support cost-effective, evidence-based medication therapy to improve patient outcomes and reduce total cost of care.

2. Participate in creating resources, references, presentations, and other updates to support the application of evidence-based medicine to direct patient care.

3. Engage with stakeholders to communicate and distribute materials that are developed to support evidence-based medication use.

4. Serve as a resource to BILHPN providers and care managers for general drug information and pharmacy policy and implement initiatives through education and support.

5. Complete comprehensive medication histories and medication reconciliation. Review and assess medication regimens with focus on safety, efficacy, adherence and cost. Screen for drug interactions, therapeutic duplications, omissions, etc.

6. Recommend adjustment in medication regimens to improve patient outcomes. Monitor patients for therapeutic and/or adverse response to medication. Provide medication counseling to patients and caregivers. Proactively assess medication adherence, and refer patients to patient assistance programs as needed.

7. Document patient care activities in electronic health record and care management systems.

8. Assist in identifying and developing interventions for BILHPN patients that are not meeting target quality measures.

9. Participate in the development and support of medication utilization initiatives that promote the cost-effective use of pharmacotherapy for patients through involvement in evaluation of market and guideline influence on pharmacy cost and utilization trends.

10. Understand and communicate the impact of pharmacy utilization on patient outcomes and total medical expense by reviewing key therapeutic areas and monitoring guidelines and/or market changes.

11. Participate in the developmment and implementation of evidence-based treatment guidelines and operational policies and procedures.

Requirements:

+ Must be a graduate of an accredited college of pharmacy.

+ Must possess Massachusetts pharmacist license in good standing at time of hire. Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP) or Geriatric Pharmacy (BCGP) , or equivalent as approved by Manager, or must obtain certification within one year of eligibility or within a reasonable timeframe as approved by Manager.

+ Requires a minimum of 2 years' experience within a collaborative ambulatory care practice or completion of a post-graduate pharmacy residency focused in ambulatory care pharmacy practice.
