A Clinical Pharmacist Specialist Oncology will initiate, modify and discontinue medications within a Collaborative Practice Agreement with oncologists as defined by the California State Board of Pharmacy. This pharmacist may also support Oncology and Pharmacy Services by managing and overseeing the prescribing, dosing modification, order entry verification, preparation, labeling, dispensing, handling, transportation, administration, and waste/disposal of all Hazardous and Non-Hazardous medications used in Oncology Services.

The pharmacist functions as a vital member of the healthcare team through independent, interdependent and dependent roles within the healthcare system. Provides age -appropriate care for infant, child, adolescent, adult, and geriatric patients.

The Clinical Pharmacist Specialty is an autonomous highly skilled Pharmacist integrated into Oncology Services so as to provide Medical Staff, Clinical staff, Pharmacy Staff, and Patient education regarding any and all medications utilized in Oncology services for the treatment of Cancer and Palliative care.

ESSENTIAL FUNCTIONS

The degree of fluency required will vary depending upon the nature of the position. For direct patient care roles: Performs and maintains currency of essential competencies as required by specific area of hire and populations served.

* Initiate, modify and discontinue medications used to treat cancer patients.

* Time Management so Oncology Pharmacy Responsibilities are in alignment with patient need and scheduling of Oncology Services.

* Professional Demeanor and interactions with all colleagues and patients.

* Interpretation and understanding of patient specific medication order.

* Proficient at Pharmacy order entry, verification of CPOE, and familiar with RN and MD charting within EMR.

* Exceptional communication with all healthcare providers for the sake of safe provision of Chemotherapy.

* Collaboration ration and oversight of the Oncology Pharmacy Technician(s) in order to maintain an environment of Medication Safety.

* Accuracy, Comfort, and Skill managing Chemotherapy plan templates within an EMR.

* Collaboration with Oncology Services Leadership, Medical Staff, and Hospital Pharmacy Leadership to maintain and facilitate financial stewardship and clinical excellence/care.

EXPERIENCE

Required/Preferred Minimum Experience Details

Preferred 2 years Hospital or Outpatient Oncology Pharmacy experience.

Preferred MOSA IQ, ARA, EPIC BEACON EMR experience.

JOB SPECIFIC KNOWLEDGE, SKILLS and ABILITIES

. Knowledge and familiarity with NCCN Guidelines and Oncology Care Plans.

. Knowledge and familiarity with Clinical Best Practices and Available Resources to maintain safe and efficacious medication therapies.

. Accuracy with pharmaceutical calculations such as BSA, AUC, and Dosing of all Prescribed Chemotherapeutic Agents.

. Knowledge and familiarity with Chemotherapeutic Agents Dosing, Dilution, Preparation, Contraindications, and Administration.

. Knowledge and familiarity with Regulations concerning the Sterile compounding of Non-Hazardous and Hazardous Medications.

. Knowledge and familiarity with Pharmacy Waste processes for Trace and Bulk Chemotherapy as well as Non-Hazardous Medications.

JOB SPECIFIC KNOWLEDGE, SKILLS and ABILITIES

. Familiarity and comfort managing a potential Chemotherapy or Hazardous Medication Spill and oversight of the process so as to ensure a safe Medication environment.

. Ability to maintain concentration and accuracy despite a busy environment with distractions and Medications with a narrow therapeutic window and may potentially cause harm if inappropriately prescribed, prepared, or administered.

. Comfort working in a stressful and high acuity Patient Care setting.

. Comfort and skill interviewing patients, family members, medical providers, and outpatient Pharmacy staff in an effort to facilitate medication reconciliation.

LICENSES and CERTIFICATIONS

Licenses, Certifications and Registrations must be unencumbered and valid in the state(s) of hire and wherever care is delivered.

  • Licensed by the California State Board of Pharmacy as a Registered Pharmacist or able to obtain license before starting position
  • Advanced Practice Pharmacist license may be required from BOP within 6 months of starting in the position
  • ACLS required on hire or within the introductory/training period or no later than 120 days

PI
