Job Description:

The Clinical Pharmacist Specialist (CPS) Oncology is an autonomous highly skilled pharmacist that functions at the highest level of clinical practice to support the establishment and delivery of evidence-based pharmaceutical care services through direct and indirect interaction between patients and providers. The CPS pharmacist serves as a multidisciplinary team member and works collaboratively with other healthcare providers under a Collaborative Practice Agreement as defined by the California State Board of Pharmacy providing advanced clinical service such as initiating, modifying, or discontinuing treatments. Furthermore, the clinical pharmacist specialist ensures establishment of safe and cost-effective pharmacotherapy to our patients by managing and overseeing the prescribing, authorizing, compounding of all hazardous and non-hazardous treatments offered as oncology services. The CPS pharmacist is an independent and self-directed skilled pharmacist to act as a drug information resource to patients and other healthcare providers (medical and clinical staff) and to provide oncology related services including but not limited to disease education, counseling, and coordination of care with other multidisciplinary providers.

  • Collaborates with providers to initiate, implement, modify, or discontinue drug therapies that promote desirable (safe and cost effective) clinical outcome based on most recent literatures and guidelines
  • Collaborates and oversights over oncology pharmacy technicians to maintain a safe medication environment
  • Addresses and assists to resolve medication related questions/issues that arise in the oncology specialty area
  • Proactively collaborates with other team members and acts as a coordinator between different teams and work with specialty pharmacies to ensure timely alignment with patientsÂ’ access to oral and IV drug therapy
  • Utilizes clinical knowledge to recommend, design, implement, monitor, and modify pharmaceutical care plans when appropriate according to guidelines and/or established protocols at Providence St. Joseph Health in a timely manner to optimize patient outcome
  • Works closely with physicians, nurses, and other staffs to provide clinical pharmacy support when caring for patients and recommends and documents interventions when applicable
  • Stays up to date with changes in treatment guidelines and new approved oncology related drugs therapies and educates multidisciplinary team members regarding managing and monitoring of oncology therapies and agents
  • Collaborates with research team members to participate in cancer related clinical trials performed by investigators and providers at Providence St. Joseph Health
  • Works with management team to perform formulary review of oncology drugs and presents to pharmacy and therapeutic (P&T) and other oncology related committees at Providence St. Joseph Health
  • Addresses medication errors and near misses to the management team and works closely with managers and other team members to implement system changes to resolve issues and ensure patient safety
  • Works with other team members and leadership team to develop and implement standard work processes and oncology related protocols to address possible systems gaps and improve quality of patient care and safety
  • Collaborates with oncology services leadership, medical staff, and hospital pharmacy leadership to maintain and facilitate financial stewardship and clinical excellence/care
  • Performs and maintains currency of essential competencies as required and defined by the health care team
  • Comfort and skill in interviewing patients, family members, medical providers, and outpatient pharmacy staff to facilitate medication reconciliation
