Summary The Cincinnati OH VA Medical Center is seeking well-qualified candidates for Pain Clinical Pharmacy Specialist. Responsibilities Responsibilities and duties include but are not limited to: The Clinical Pharmacy Specialist (CPS) in Pain Management is an advanced practice provider authorized under a scope of practice to autonomously prescribe and provide comprehensive medication management services under a Scope of Practice which may include prescriptive authority, monitoring drug therapy, ordering laboratory tests, and other specified clinical functions. S/he is a licensed professional with knowledge and proficiency in pharmacy practice. The CPS is the expert in drug therapy and functions as a consultant to other health care professionals. The CPS will be responsible for clinical and administrative management of the pain management services provided by the Pharmacy Service. The CPS will act as the primary liaison between Pharmacy Service and the assigned area(s). The CPS is under the supervision of the PACT Pharmacist Supervisor but also receives basic program guidance and oversight from the Rehab Careline. The outpatient pain management clinical pharmacy specialist may be located primarily at the Cincinnati VAMC Vine Street location but may also be assigned at the Fort Thomas Division, , and/or any of the community-based outpatient clinics (CBOCs). Duties and responsibilities include but not limited to: Responsible for pain management pharmacy services for assigned Veterans. Evaluates, prescribes, modifies, or discontinues medication therapy and acts as a consultant for pain management pharmacotherapy working under a scope of practice and care coordination agreement. Performs rating scales on pain control, symptoms, and functional status Completes clinical reminders and screenings. Provides overdose education and prescribes naloxone when needed Uses available population health tools to identify and address risk mitigation needs focused on safe pain care. Screens for substance use disorders when indicated, performs brief interventions when indicated, including addressing aberrant behaviors. Serves as a subject matter expert resource in pain management pharmacotherapy for the Medical Center. Works with patients who have been identified as having compliance/adherence issues with their medications Encounters, assessing barriers to compliance, and identifying and educating on ways to improve compliance Works directly with appropriate services and providers to help ensure continuity of care for patients. Reviews and evaluates requests for non-formulary and restricted drugs (prior authorization) for appropriateness and compliance with established VA National Formulary, current treatment guidelines, criteria for use, and medication use initiatives. Works with providers to ensure compliance with national, VISN, and local initiatives, and promotes and monitors compliance for established drug therapy policies. Serves as an authority on drug usage, interactions, over-dosages, and medication compliance to facility staff. Uses appropriate references to research drug information for providing timely and accurate responses to drug information questions. Participates in the focused and ongoing professional practice (FPPE & OPPE) evaluation process. Attends, participates, and/or contributes as assigned as an active member on a committee, work group; task forces, etc. with a focus on ensuring quality in clinical services. Monitors for and reports medication errors, adverse drug reactions, allergies and patient compliance issues and documents findings per local policy. Serves as a primary clinical preceptor for pharmacy trainees. Provides educational programs to pharmacy and non-pharmacy staff as needed. Assesses and provides educational needs of the patient and family to help ensure that pharmaceutical outcome goals are met. Designs and/or participates in research activities involving drug therapy or outcome measures. Completes all assigned training by the due date. Participates in self-development activities to maintain and improve professional competency. Work Schedule: M-F 8:00AM-4:30PM Virtual: This is not a virtual position. Qualifications Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60; phone: , or through their Web site at: (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. § 7402(f)). Effective November 30, 1999, this is a requirement for employment. Exception. Non-licensed pharmacists who otherwise meet the eligibility requirements may be given a temporary appointment at the entry level as a Graduate Pharmacist under the authority of May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). GRADE REQUIREMENTS Creditable Experience (1) Knowledge of Professional Pharmacy Practices. To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Professional practice means paid/non-paid employment as a professional or unlicensed graduate pharmacist as defined by the appropriate licensing board. (2) Residency and Fellowship Training. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for-year basis. The pharmacy residency program must be accredited by the American Society of Health-System Pharmacists (ASHP). A fellowship program that is not accredited by the American College of Clinical Pharmacy (ACCP) will need to have comparable standards for experience to be creditable (Professional Standards Board refers to the Deputy Chief Consultant for Professional Practice for the determination). (3) Quality of Experience. Qualifying experience must be at a level comparable to pharmacy experience at the next lower level. Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist (under supervision) and subsequently passed the appropriate licensure examination. (4) Part-time Experience. Part-time experience as a professional pharmacist is credited according to its relationship to the full-time workweek.
