Description
The responsibilities entail providing ambulatory care clinical pharmacy services including but not limited to the evaluation, interpretation and management of medications to improve the outcomes and quality of healthcare to patients through the use of value-based therapeutics. The activities entail collaborating with physicians and other health care professionals and applying medication management strategies in alignment with evidence-based medicine.

ESSENTIAL FUNCTIONS

* Consistently exhibits behavior and communication skills that demonstrate Optum's commitment to high levels of customer service to internal and external customers.
* Serves as a role model by taking initiative to meet department/team objectives
* Execute disease state management programs targeting chronic conditions such as anticoagulation therapy, heart failure, diabetes, hypertension and other diseases
* Appropriately manage, assess and/or recommend evidence based medication therapy based upon the patient's clinical needs for optimizing care outcomes and reducing any potential patient safety problems (i.e. drug interactions, suboptimal medication regimens, high risk medications etc.). Analyze medication history to formulate clinically sound recommendations through medication reconciliation.
* Demonstrates appropriate judgment on conditions requiring consultation, referral or evaluation by patient's PCP or other health care professional.
* Educates patients and/or families about preventative care, medical issues and use of prescribed medications to enhance healthy behavior in alignment with disease management program goals and objectives
* Addresses new patient referrals, labs, tasks, patient follow-up and drug information questions in a timely manner and consistent with department policy and procedures.
* Fully documents all aspects of patient care, and completes all required paperwork for billing and administrative purposes to meet business and compliance requirements.
* Utilizes pharmacy data and other reports to develop and implement strategies to improve quality measures
* Maintain productivity standards through the ability to multi-task to complete tasks in a thorough and timely fashion.
* Participates in the development of treatment algorithms, drug utilization management strategies, intervention techniques and policies and procedures needed to support the Optum pharmacy programs.
* Ability to communicate and delegate responsibilities to other teammates for optimization of time and resources
* Maintain competency by participating in and pursuing professional development activities which align with the expanded services provided by the department (including but not limited to chronic disease management, transitions of care, quality initiatives, cost effective prescribing, and education for clinicians)
* Regularly communicate and update physicians and other health professionals about medication-related information and pharmacy department guidance at center meetings and 1-on-1 settings
* Align and build effective working relationships with other health care teammates in demonstrating the ability to work in a cross functional team environment
* Participates in Optum committees or meetings relevant to the functions of the pharmacy services department, when appropriate.
* Assist in clinical instruction and supervision of medical assistants, certified pharmacy technicians, pharmacy students and others as required
* Uses, protects, and discloses HCP patients' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards.
* Performs additional duties as assigned.

EDUCATION

* Current California State Pharmacy License and Pharm.D.
* ASHP-accredited residency in Pharmacy Practice or equivalent background/experience
* Board certification in pharmacy specialties preferred (i.e. Ambulatory Care-BCACP, Pharmacotherapy-BCPS).

EXPERIENCE

* At least 3 years' experience in clinical pharmacy practice including clinical pharmacotherapies, disease state management and drug use evaluations OR completion of 1 or 2 year accredited pharmacy residency program.

STRONGLY PREFERRED

* Hospital residency
* Ambulatory care experience

KNOWLEDGE, SKILLS, ABILITIES

* Float between Los Angeles and Montebello, CA
* Ability to communicate verbally and in writing in a clear yet impactful manner
* Knowledge of Microsoft software.
* Expertise and current knowledge in clinical pharmacology, disease state management, practical application of ambulatory care pharmacy practice.
* Excellent organizational, interpersonal and decision-making skills.
* Ability to work independently and manage multiple, concurrent projects for completion in a thorough and timely manner
* Ability to analyze data and formulate sound conclusions and recommendations
* Knowledge of pharmacy benefits such as drug formularies with general understanding of pharmacoeconomics
