Summary The VA Northeast Ohio Healthcare System is recruiting for a Clinical Pharmacy Specialist (Ambulatory Care). The Clinical Pharmacy Specialist (Ambulatory Care) will function in Pharmacy Service. Responsibilities The duties of the Clinical Pharmacy Specialist (Ambulatory Care) include, but are not limited to: The primary functions are managing therapy of patients with chronic medical conditions to achieve predetermined therapeutic goals; responding to consult requests from medical staff; improving success rate on critical medical center performance measures; educating and training medical and allied health care staff on medication therapy and disease management guidelines, pharmacy Plans and provides patient related clinical pharmacy services in assigned clinics. Functions under a scope of practice consistent with skills, training, and education, and consistent with the pharmaceutical care needs in ambulatory care. Functions within defined scope of practice and therapy guidelines to achieve therapeutic goals in patients with hypertension, diabetes, lipid disorders, cardiac disorders, and as otherwise assigned. Effectively addresses patients who have not achieved therapeutic goals because of poor adherence to prescribed regimen, adverse effects, polypharmacy, and/or suboptimal medicine regimen. Achieves therapeutic goals by monitoring response to prescribed therapy via appropriate history, physical and laboratory monitors, as well as prescription refill evidence and feedback from patient. Orders appropriate laboratory tests at appropriate times, as necessary, to determine response and safety to prescribed therapy. Reviews patient profile for appropriateness, duplication, drug/drug and drug/food interactions, incompatibilities, or other drug related problems, and either makes appropriate changes to patient's medications or recommends change to the provider who is seeing the patient. Takes responsible and appropriate actions to promote optimum, most cost effective, drug therapy to patients under their care. Performs medication reconciliation of patient's profiles and medications being taken, and updates medication profile as necessary. Renews prescriptions for maintenance medications as needed and issues new prescriptions as appropriate and within scope of practice. Evaluates and responds to consult requests for assigned prior authorization medications within 96 hours. When assigned, incumbent participates in various primary care and specialty clinic areas, as part of an interdisciplinary team to provide pharmaceutical care needs. Attends meetings and provides backup coverage at various other CBOCs, as required. If assigned to a CBOC without a Pharmacy on site, the incumbent provides oversight to the medication distribution process in designated clinic(s), including dispensing functions when requested and deemed appropriate Work Schedule: Monday through Friday 8:00 a.m. - 4:30 p.m. Compressed/Flexible: Not Available Telework: Not Available Virtual: This is not a virtual position. Qualifications Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Note Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy. Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d) May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-13 Clinical Pharmacist Experience: In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level? Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: -Ambulatory Care experience with direct patient care -PGY-1 Residency with Ambulatory Care focus or PGY-2 Residency in ambulatory care -Experience precepting Pharmacy Trainees and Residents -Board of Pharmacy Specialties Certified and/or Certified Diabetes Educator -Experience serving as primary provider in shared medical appointments -Experience leading formulary initiatives -Experience with research design, writing, presentation References: VA Handbook 5005, Part II, Appendix G15 Licensed Pharmacist Qualification Standard GS-660 Physical Requirements: Incumbent must be physically and mentally able to perform efficiently the duties of the position. The work requires regular and recurring physical exertion, involving walking and standing for prolonged periods of time, lifting/carrying of pharmaceutical supplies weighing at least 50 lbs., and bending and stooping. The incumbent must have a current, valid driver's license and must be able to drive in all forms of inclement weather conditions. (Although the incumbent is assigned to a specific location, it must be understood that as an employee of the Cleveland VA Medical Center, the possibility exists for reassignment to a different unit in accordance with good management practices.)
