Overview

This Clinical Pharmacy Specialist will reside in the Indiana University Health LifeCare Clinic located at Methodist Hospital and join two additional pharmacists as well as a dedicated pharmacy technician to complete the pharmacy team. This position provides an opportunity to participate in a multidisciplinary team providing pharmacy support as well as serving as a strong patient advocate for people living with HIV across the state.

LifeCare is one of the largest providers of HIV medical services in the state of Indiana. For more information about our services: . IU Health serves as a teaching site for the Colleges of Pharmacy at Butler University, Manchester University, and Purdue University as well as Indiana University School of Medicine. IU Health Academic Health Center is a teaching site for 16 PGY-1 pharmacy residents as well as many PGY-2 pharmacy residents, including Infectious Disease and Ambulatory Care.

Services Provided by Clinical Pharmacy Specialist, HIV

+ Conduct independent and/or collaborative patient appointments to improve outcomes related to drug therapy

+ Assist with initiation and modification of antiretroviral therapy (ART) regimens

+ Participate in antiretroviral stewardship to decrease medication errors and improve outcomes during inpatient admissions

+ Coordinate with the Clinical Pharmacy Technician to ensure procurement of antiretroviral therapy (ART) regimens

+ Provide expertise and leadership for implementation of drug policy, guidelines, protocols, and clinical decision support

+ Participate in departmental and interdisciplinary committees

+ Develop strategies to continuously improve the quality and provision of pharmacy services

+ Ensure compliance with quality and regulatory standards

+ Precept PGY-1 and PGY-2 residents as well as students on the HIV elective rotation

+ Work closely with IU Health Pharmacy Retail teams to streamline pharmacy services

Requirements

+ Doctor of Pharmacy degree

+ Required: PGY-1 Pharmacy Residency

+ Required: PGY-2 Pharmacy Residency (preferably in HIV or Infectious Disease) or equivalent practice experience

+ Preferred: Board Certification

+ Preferred: HIV Pharmacist™ (AAHIVP)

+ Preferred: Academic Health System experience

This position exists to provide comprehensive, value based, medication-related care to achieve definite outcomes that improve a patient's quality of life. The incumbent acts in collaboration with healthcare professionals in the shared responsibility for patient drug therapy outcomes. Key job duties may consist of ensuring safety and effectiveness of the medication use process, processing and dispensing medications per order or prescription, medication therapy monitoring, regulatory compliance, maintaining oversight over pharmacist extenders, maintaining the security of medications, and protecting the confidentiality of patient information. Additionally, is competent at providing drug information support for medical staff, ensuring continuity of care, and identifying potential challenges in common situations.

• Requires current Indiana License or eligibility for Indiana licensure as a pharmacist.

• Successful completion of an ASHP accredited PGY1 and/or PGY2 pharmacy residency may be required.
