Description
Participates in education and service in general clinical pharmacy or in a specialty area (Pharmacy Research Fellows who are 100% dedicated to Outcomes Research and Pharmacoeconomics also may be included). Routinely interacts with physicians and nurses to make recommendations regarding drug therapy of patients under the tutelage of a clinical pharmacy preceptor. Receives education and training in various areas of clinical pharmacy and will also serve to educate staff pharmacists, supportive personnel, and pharmacy students, as well as other allied health professionals, nurses, and physicians in pharmacotherapy. Receives education in providing drug information and pharmacy and therapeutics activities, as well as participating in supporting clinical research.

Responsibilities:

* Performs pharmacotherapeutic monitoring and provision of pharmaceutical care of patients in the area of service responsibility in a routine continuous manner taking into account the age and clinical condition of each patient.
* Provides and documents pharmacotherapeutic recommendations that positively affect patient outcome and documents all activities through departmental reports and quality assurance mechanisms.
* Learns how to demonstrate a service orientation toward patients, staff, and visitors.
* Provides general and patient specific drug information to patients, physicians, nurses, and other health care professionals. Interacts routinely with physicians and nurses to provide optimal pharmaceutical care to patients.
* Participates in and supports Pharmacy and Therapeutics Committee activities, such as drug use evaluation, adverse drug reaction monitoring, formulary review, and maintenance and assistance in monograph preparation for drug addition to the formulary.
* May serve as an experiential rotation preceptor for pharmacy students and staff pharmacists.
* Does not have direct patient care activities or pharmacist responsibilities unless appropriately licensed.
* Other duties as assigned.

Education:

* Candidates must be completing a Doctor of Pharmacy degree.
* Pharmacy Research Fellows must be completing a Doctor of Pharmacy degree, Master of Science degree or PhD.
* For PGY-2 residents, certificate of completion for post-graduate year 1 residency (PGY-1) required within 30 days.

Certifications:

* Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current State within 90 days of being hired as a Clinical Pharmacy Resident.
* New hires who do not hold a current Ohio Registered Pharmacist (RPh) license, are require to obtain an Ohio Pharmacy Intern license and maintain until the Ohio RPh license is active.

Complexity of Work:

* Within scope of job, requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
* Must be able to work in a stressful environment.

Work Experience:

* None required.

Physical Requirements:

* Requires full range of motion, manual and finger dexterity and eye-hand coordination.
* Requires standing and walking for extensive periods of time.
* Requires corrected hearing and vision to normal range.

Personal Protective Equipment:

* Follows standard precautions using personal protective equipment as required.
