Novant Health is seeking a Clinical Pharmacist, to join our outpatient oncology pharmacy team, to deliver optimal medication management across all dimensions of care, every time.

Essential functions include:

This Clinical Pharmacist-Hematology/Transplantation and Cellular Therapy will be an integral member of a multidisciplinary team optimizing collaborative, patient-centered care. Post-graduate oncology pharmacy practice residency (PGY2 Oncology) is required along with experience working with a Hematology and TCT team. Duties include medication management and monitoring, treatment plan review, post-transplant vaccine administration, patient counseling, assistance in transitions of care among daily clinic duties. In addition, participation in research and quality improvement including but not limited to policy/guideline development and institutional research activities.

* Clinical review, entry and modification of oncology treatment plans.
* Oncology order verification, hazardous and non-hazardous sterile admixture check.
* Partnership with clinic providers and nursing to provide remarkable patient care.
* Demonstrating proficiency with equipment and software.
* Precepting of residents (PGY1/PGY2) and students.
* Participation in committees such as P&T, tumor boards and shared governance.
* Order set review and development.
* Medication therapy management and patient counseling.

Qualifications

* Education: Bachelor's Degree in Pharmacy Required;Doctorate of Pharmacy Preferred.
* Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy Required. Oncology/Hematology experience strongly preferred.
* Licensure/Certification/Registration: Current Pharmacy licensure in appropriate state(s) Required or must be completed within 30 days of start date by either examination or reciprocity. Board of Pharmacy Specialties Certification or other applicable certification Required or must be obtained and continuously maintained within 2 years of start date as applicable to the role; excludes PRN team members who may not be eligible to take certification exam.
* Additional Skills Required: Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team.
