Job Summary
Full Time/Weekdays and Weekends. Based at Carolinas Medical Center/Levine Children's Hospital, but may involve travel to other facilities as needed.
Atrium Health has an immediate Clinical Pharmacist opening for the Antimicrobial Support Network (ASN). The ASN is a comprehensive antimicrobial stewardship program focusing on enhancing antimicrobial use throughout the Charlotte Region Atrium Health facilities. The ASN provides antimicrobial stewardship and infectious disease pharmacy clinical services for 16 acute care facilities and recent expansion into the ambulatory care setting.
The ASN Clinical Pharmacist provides pharmaceutical care through patient review, clinical rounds, and clinical projects/initiatives. Activities include review of orders for medication therapy and monitoring this therapy for drug choice, dosage, duration, drug interactions, adverse drug reactions, and formulary adherence. The ASN Clinical Pharmacist also serves as a drug information resource to nurses, pharmacists, physicians, and other healthcare teammates. Essential Functions
* Designs, recommends, and monitors patient specific pharmacotherapeutic regimens with continued evaluation to determine presence of medication related problems.
* Participates in the provision of optimal medication management through a variety of means including interprofessional clinical rounds, direct patient or provider consultation, and ASN staffing.
* Involvement in longitudinal project work to benefit the Atrium Health Division of Pharmacy enterprise-wide including (but not limited to) collection of statistical data, creation or adaptation of procedures requiring improvement, and providing education for pharmacy teammates and other healthcare professionals.
* Implements recommendations effectively and utilizes the existing clinical intervention policies and procedures and documents in the medical record using the appropriate format.
* Retrieves and evaluates relevant biomedical literature to formulate appropriate responses to drug information requests in a timely and concise manner.
* Provides accurate shift to shift communication regarding unresolved situations requiring future action.
* Precepts pharmacy students and pharmacy residents.
* Checks the accuracy of the work of pharmacy support team members or others in accordance with the organization's policies and procedures.
Physical Requirements
Works primarily in the pharmacy. Delivers medications to nursing units. Requires considerable walking, standing, pushing mobile cart of 50 pounds maximum and lifting 25 pounds of material. May be required to frequently stretch, bend, squat, or kneel to perform job. Utilizes appropriate personal protective equipment (PPE) to minimize exposure to hazardous agents. Takes necessary precautions to avoid inadvertent needle sticks, minor cuts or bruises. Exposed to various conditions throughout the workday such as walking on hard surfaces, climbing stairs and changes in temperature and humidity. Must be able to perform manipulative skills such as writing, typing, and data entry into the computer and automated dispensing machines. Must have use of both hands, be able to see clearly and read small type such as that on unit-dose medication packages and syringes. Must be able to speak and be understood by others to communicate effectively. Must be able to hear normal sounds with moderate background noises. Involves rotating shifts.
Education, Experience and Certifications
Bachelor's Degree in Pharmacy or a Pharm D. from an accredited school of pharmacy required. Licensed in the state of North Carolina or eligible for reciprocity required. Successful completion of a PGY2 Infectious Diseases Pharmacy Residency or PGY1 Pharmacy Practice Residency (or equivalent years of clinical pharmacy experience) with previous antimicrobial stewardship experience strongly preferred. Board certification or board eligible preferred. Highly effective verbal communication and interpersonal skills with patients, visitors, and other healthcare team members to establish working relationships that foster optimal quality of patient care required.
