We are looking for a Clinical Pharmacist Specialist for the Pharmacy Department at Queen of the Valley Medical Center.
Location: Napa, CA
Work Schedule: Full Time - 80 Biweekly Hours
Shift: 10-hour, Days
Job Summary:
Reporting to the Director of Pharmacy, this position is responsible for providing and coordinating pharmaceutical care services and collaborative drug therapy management for oncology patients. The oncology Clinical Pharmacist Specialist will serve as a resource to patients, physicians, pharmacists, nurses and other members of the health care team serving the oncology patient population.
Essential Functions:
* Implements and maintains established department, unit, and hospital policies, procedures, and objectives as well as improvising organization performance program, safety, environmental, and infection control policies.
* Demonstrates excellent customer service and positive interpersonal relationships with physicians, staff, and patients/ families.
* Actively participates in daily patient care to identify patients for therapeutic monitoring and to discuss potential interventions.
* Provides comprehensive pharmacy services to patients, including drug therapy assessment based upon evaluation and interpretation of patient history, age medical condition, surgical history and objective physical biochemical data development of therapeutic goals and endpoints of drug regimen preparation of discharge orders and patient education within scope of practice.
* Manages drug therapy protocols as a part of an interdisciplinary team. Collaborates with multidisciplinary team to assist with problem identification and resolution, cost containment issues, implementation of new services, and system/ performance measures.
* Prospective involvement in medication therapy decisions including collaborative drug therapy practice. Reviews, interprets and evaluates medication therapy to identify, prevent, and resolve potential or actual drug related problems.
* Develops, implements and evaluates programs, or area services and initiatives. Leads development of practice standards and measurable outcomes based on performance evaluation and research. Collaborates on investigational drug studies, as necessary.
* Monitor drug therapy and suggests alternative drug therapy when appropriate.
* Coordinates ongoing quality assessments and improvements of clinical pharmacy activities in collaboration with physicians and implements necessary action as appropriate.
* Effectively communicates verbally and/or in written pharmacy functions to the interdisciplinary team.
* Serves as a drug information resource person to physicians, patients, and other health care staff. Promotes evidence base therapeutics. Documents interventions and their outcomes for use in drug therapy quality improvement efforts.
* Prepares clear and concise clinical briefs for use by nurses, medical staff, and pharmacy.
* Acts as liaison and consults with nurses and medical staff concerning prescribing, transcribing, dispensing. Administration/ monitoring of medications; reports patient adverse drug reactions, medication variances and complaints; participates in education activities and programs for the interdisciplinary team.
* Supervises employee directly to ensure safe, legal, and effective drug distribution and use.
* Participates in QA review; regularly documents inspection of all medication storage areas.
* Checks stock on regular basis to identify and reorder outdated stock and ensure that stock is maintained in accordance with manufacturer requirements.
* Ensures secure storage of narcotics and other controlled substances on a regular basis and verifies inventory control.
* Interprets and clarifies orders for all types of medications.
* Ensures delivery of quality pharmacy services for inpatients, outpatients in accordance with established legal requirements.
* Compounds, labels, and packages medications, and pharmaceuticals meeting professional standards and guidelines.
* Performs or oversees the preparation of chemotherapeutics and other preparations for IV administration.
* Maintains and engages professional growth and development through participation in educational programs with emphasis on oncology, current literature in-service meetings, and workshops; demonstrates a comprehensive clinical pharmacy knowledge base.
* Attends meetings and participates on committees as directed/required.
* Serves as a preceptor to pharmacy residents and students when applicable.
Skills:
* Well versed in USP 797, 800, and 795.
* Meets the Pharmacy Department specified competency requirements consistent with a post graduate year 1 residency (PGY1) and post-graduate year 2 (PGY2) in oncology.
Minimum Position Requirements:
Education: Graduate from an accredited school or college of pharmacy.
Experience: 5 years oncology experience.
Licenses/Certifications: Current California Registered Pharmacist License in good standing.
Preferred Position Qualifications:
Education: Course/Training - Post graduate year 1 residency (PGY1) and post-graduate year 2 (PGY2) in oncology.
Licenses/Certifications: Board Certified Oncology Pharmacist Specialist (BCOP) or within 2 years of hire.
