CLINICAL PHARMACIST SPECIALIST - INTENSIVE CARE UNITS (ICU) :

The Clinical Pharmacist Specialists are responsible and accountable for the provision of safe, effective, and prompt medication therapy. Through various assignments within the department, they provide support of centralized and decentralized medication-use systems as well as deliver optimal medication therapy to patients with a broad range of disease states. Clinical Pharmacist Specialists proficiently provide direct patient-centered care and integrated pharmacy operational services in a decentralized practice setting with physicians, nurses, and other hospital personnel. These clinicians are aligned with target interdisciplinary programs and specialty services to deliver medication therapy management within specialty patient care services and to ensure pharmaceutical care programs are appropriately integrated throughout the institution. In these clinical roles, Clinical Pharmacist Specialists participate in all necessary aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care to the patients in their assigned areas. They will serve as departmental resources and liaisons to other departments, hospital personnel, or external groups. The Clinical Specialists will also provide medication and practice-related education/training and actively serve as preceptors for doctor of pharmacy students and pharmacy residents.

Day shift position, may occasionally need to cover evenings pending departmental needs.

MINIMUM REQUIREMENTS:

EDUCATION: Bachelor's Degree Pharmacy, PharmD, or Bachelor's of Science degree with equivalent decentralized hospital pharmacy experience (3years)

EXPERIENCE: Minimum requirement of 1-2 years Clinical Pharmacy Pharmacy practice that is consistent with the area of specialty in the job posting or 1-2 years Completion of an ASHP-accredited PGY1 pharmacy residency or Board Certification in related area of practice.

SPECIALIZED KNOWLEDGE: This position requires advanced knowledge in intensive care medicine. This position will work closely with ICU teams and related service lines which will require strong clinical, communication, organizational, and teamwork skills.
