Summary:

+ Responsible for provision of pharmaceutical care including medication therapy management (MTM), medication regimen review, and medication adherence support for patients in the outpatient practices of Pennsylvania Hospital. Evaluate, monitor and modify drug therapy, and provide recommendations to optimize patient outcomes and maximize safety. Facilitate transitions to ensure medication regimens are managed and monitored across various levels of care. Serve as a liaison with the outpatient pharmacy to ensure patients receive prescribed medications. Provide medication counseling for patients and families. Coordinate pharmacotherapy education of pharmacists, residents and students, and provide drug information to medical staff. Develop programs, policies, and guidelines that support evidence-based standards of practice. Recommend and implement community outreach initiatives. Serve as an active member of hospital and health system committees. Provide support for safe and effective use of information technology. Contribute to clinical medication management through quality initiatives and ensure cost effective pharmacotherapy.

Credentials:

+ Bachelor's Degree in Pharmacy or Doctor of Pharmacy Degree from an accredited college of pharmacy , completion of a Pharmacy Practice Residency (PGY-1) and current pharmacy license to practice in the state of Pennsylvania required. PGY-2 residency in related field, board certification or equivalent experience preferred
