Location: Santa Rosa, CA

Work Schedule: Full Time - 80 Biweekly Hours

Shift: 10-hour, Days

**Job Summary:**

Provides the professional didactic knowledge, clinical and technical skills required by the Department to assure that every patient receives safe and effective drug therapy during hospitalization. Works independently. Requires little supervision. Seeks supervisory direction as required.

**Essential Functions:**

+ Oversees the operation of the department on a per-shift basis to include the dispensing process, IV admixture compounding, unit dose cart fill, and repackaging operations.

+ Assures personnel carry out assigned duties, monitoring shift arrival and departures for technical/distribution staff. Assumes responsibility for department operations when designated by the Director or Manager.

+ Interprets physician orders for correctness, reviewing for possible drug interactions/incompatibilities. Transcribes new orders into information system, documenting clarification of orders with physicians, and potential adverse drug events. Documents clinical interventions.

+ Maintains narcotic inventory reconciling same to narcotic tracking system. Performs assigned floor checks, maintains emergency trays, and fills outpatient prescriptions as required.

+ Assists in maintaining hospital records as required by State/Federal/Regulatory requirements. Maintain standard of practice as determined by the American Society of Health System Pharmacists and Joint Commission of Accreditation of Healthcare Organizations.

+ Communicates with physicians/nurses/other health care professionals providing pharmaceutical expertise as required. Utilizes available literature sources to give drug information as requested or required.

+ Maintains working knowledge of pharmacy and nursing medication policies.

+ Assists in classification of Medication Utilization Evaluation and Adverse Drug Events data. Gains awareness of Quality Improvement goals and assistance in related programs and surveys.

+ Supports Director or Manager in implementation of departmental and hospital goals. Provides orientation of new employees and in-service training to pharmacy and hospital personnel as determined by the Director/Manager.

+ Performs chart reviews as required for assessment of therapies, interactions, and patient response to medications and for regulatory required monitoring of therapy parameters.

**Skills:**

+ Working knowledge of:

+ The practice of institutional pharmacy.

+ Pharmacy reference materials and ability to utilize them in the provision of drug information.

+ Performance of research utilizing on line resources/internet.

+ Computer systems in on-line information retrieval.

+ The practice of inventory/stock controls.

+ Drug/solutions/delivery systems.

+ Drug interactions/incompatibilities.

+ Current laws/regulations which affect practice of pharmacy.

+ Aseptic compounding techniques.

+ The preparation of extemporaneous sterile/non-sterile products.

+ Ability to:

+ Utilize computers and basic word processing and spreadsheets.

+ Plan/direct organize/delegate effectively during shift.

+ Effectively communicate in written/verbal mediums with staff/health care personnel.

**Minimum Position Requirements:**

**Education:** Coursework/Training - Graduate of an accredited School of Pharmacy.

**Experience:** Hospital/health care experience; entry level individuals will be considered.

**Licenses/Certifications:** Current license to practice pharmacy in the State of California.

**Preferred Position Qualifications:**

**Education:** Coursework/Training - Pharmacy Residency.

**Experience:** 1 year Hospital pharmacy experience.
