General Description of the Job Class: This clinical pharmacist will mai ntain an oncology clinical practice inthe Duke Cancer Center.Duties and Responsibilities of this LevelClinical Pharmacist Responsibilities Clini cal Practice and Medication Management: # Participate in clinic and prov ide clinical pharmacy services andoversight for safe use ofmedication. T his includes prescribing, dispensing, administering, andmonitoring of me dications.This may include, but is not limited to:o Perform chemotherapy counseling/educationo Complete/Review medication review/reconciliationo Provide medication counselingo Interpret pharmacokinetic/therapeutic dr ug monitoring datao Screen for drug interactions, therapeutic duplicatio ns, omissions,etc.o Provide initial screen and evaluation of chemotherap y orders forappropriatenesso Monitor patients for therapeutic and/or tox ic response tomedication/chemotherapyo Evaluate and address supportive c are needso Provide oral chemotherapy counseling and adherence tools andfollow-upo Coordinate patient care withinpatient colleagues# Ensures phar macy staff competency so that pharmacists are providingappropriate drugt herapy withconsideration of the patient's diagnosis, age, weight, allerg ies, ADRs,organ function, and diet.# Provides support toimprove safety and to reduce risk to patientstreated in the Program.Reportsevents and/o r concerns to SRS system.# Provides oversight of cost-effective use of m edications andtreatments. This includesriskreduction and correct utiliz ation of resources to optimize care.# Provides coordination and oversigh tof transition of high-acuity treatments from inpatient toambulatory inf usion center.# Provides leadership of developing guidelines, order forms , andcritical pathways toimprove thecare and safety of patients.# Provid es leadership to maintain TJC accreditation standards withinthe realm of medication usewithin the Program.# Provide guidance and educational supp ort to all pharmacy staffproviding support topatients# Represent the area specific concerns for Information Technologyapplications.# Participate in oncology grand rounds, tumor boards, patient-carerounds, etc.# Coord inate with Duke Retail pharmacies to ensure medicationadherence.# Collab orates with Infusion Pharmacy staff to provide continuity ofcare on medi cationrelated issues.Research Practice Management:# Provide support of c linical trials education of Program staff toassure researchcompliance. # Provide education to participants in oncology clinical trials.# Develop and supportstudy protocol development. This includes the development o f conceptsheets,in conjunction with the Investigational Chemotherapy Pha rmacist, forpharmaceutical-sponsored,investigator-initiated studies and other protocols with oncologyfaculty members. # Develop and participate in pharmacy-related research projects andquality improvementinitiatives, MUEs, etc. within the Program. # Provide oversight of pharmacy staff imp lementing drug-relatedclinical research within theProgram. When opportun ity arises, serves on the IRB, IRB AuditSubcommittee, or CPC Protocol Re viewand Monitoring Committee. Educational Practice Management:# Provide oversight of oncology staff education for critical elementsof medication use inpatientcare and clinical research.# Educates medical, nursing, an d pharmacy staff about the management,prescribing andmonitoring ofoncolo gy therapies including chemotherapy agents.# Participates in the educati on of pharmacy students and pharmacyresidents# Coordinates effort to pro vide timely drug information to the Program.# Providesleadership support for research efforts in patient careareas.# Assures that staff is compe tent and compliant in providing adequatepatient educationpertainingto dr ug therapy within the Program.# Presentsscholarly materials at educatio nal conferences for theoncology service areaand pharmacyprofessional mee tings and organizations (Duke, State/Local, National) Administrative Pra ctice:# Participates in departmental and hospital committees (ie. Minimum Qualifications

Education

Doctor of Pharmacy Degree OR BS Pharmacy Degree

Experience

With a BS Pharmacy degree, two years of pharmacy practice experience, or acceptable board certification, or completion of an accredited residency program and demonstrated ability to achieve positive patient outcomes in a similar environment is required. With Doctor of Pharmacy degree, no experience is required.
