Job Summary
Provide specialized pharmaceutical services to improve drug usage and therapeutic outcomes including, but not limited to, advising physicians on issues concerning drug therapy, the inherent toxicity of drugs and their side effects, as well as assisting in the prescription of appropriate doses.
Maintain liaison relationships with medical and nursing staff; provide timely information pertaining to pharmaceutical supplies, drug usage and compatibility, state and federal regulations regarding drug controls, and Joint Commission Standards.
Individualize medication regimens using sound principles, accounting for pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism and elimination with responsibility for establishing and continually improving the delivery of Pharmaceutical Care to patients within areas of direct responsibility and assisting others in the department with the same.
Job Responsibilities
Develops and maintains a consistent clinical pharmacy practice that provides optimal pharmaceutical support to patients of The Children's Hospital of Philadelphia in an appropriate, efficacious, safe, and cost-effective fashion.
* Participates in clinical rounds at least three times weekly.
* Reviews patient's pharmacotherapy using the hospital Information system (HIS) and the pharmacy computer system to ensure that the medications are appropriate in terms of dose, dosage form, route of administration, frequency, duration of therapy, and patient's disease state.
* Communicates with prescribers and nurses to recommend alternatives for inappropriate therapy or substitutes for non-formulary items.
* Ensures and promotes compliance with institution's formulary system and other guidelines set forth by the TSC and the Department of Pharmacy.
* Provides pharmacokinetics monitoring to all patients on the team of clinical practice.
* Writes, edits, and reviews formulary monographs for presentation to TSC.
* Assists in editing and reviewing the CHOP formulary on an annual basis.
* Implement and monitor cost containment initiatives
Provides drug information and/or presents educational programs to healthcare professionals and patients.
* Provides appropriate and timely consultative services (DI, pharmacokinetics, etc.) to physicians, nurses, pharmacists and other health care professionals regarding drug therapy. Ensures response time to inquiries to be no greater than 24 hours.
* Advises patients and family on the use of various medications: name, description, route, dosage form, duration of therapy, directions for preparation, administration, use, common side effects, interactions, contraindications, self-monitoring, storage, refill information and action to be taken if dose is missed.
* Develops and maintains didactic modules for ongoing staff development programs.
* Participates in the training of pharmacy students from the College of Pharmacy.
* Participates in the training of pharmacy staff and pharmacy residents.
* Writes a pharmacy newsletter article at least once per year.
Job Responsibilities (Continued)
Participates in hospital and departmental Continuous Quality Improvement (CQI) activities.
* Assists in maintaining records and statistics required to monitor and evaluate clinical pharmacy activities and effectiveness in accomplishing its stated objectives, (consults, interventions etc.).
* Develops and reviews annually 3-4 Drug Use Evaluations to ensure that the medication is appropriate in terms of dose, dosage form, route of administration, frequency, duration of therapy, patient's disease state, and cost effectiveness.
* Assists in data collection of ADRs in the institution.
* Utilize the principals of continuous quality improvement to analyze, and resolve problems in a systematic manner.
* Assists in preparing required reports by compiling information.
* Analyzes and evaluates continuously, the quality of service, and develops plans necessary to correct any deficiencies identified.
* Achieve patient safety and satisfaction through the use of lean principles (5S, Elimination of Waste, JIT, Standard Work, Quality, Promote and Comply with Standardized Procedures, Promote and Comply with Standard Work, Identify "waste" in ownership process and recommend solutions for implementation, correct abnormal conditions.)
* Acts as a liaison with nursing and medical staff concerning prescriptions, orders, drug and pharmaceutical detail, patient reactions, errors and complaints.
* Attends monthly unit base QI meetings enhance the appropriate effective utilization of the department for all hospital patients requiring such support.
* Investigate and analyze incident reports.
* Participates in and contributes to regularly scheduled or Ad Hoc Medical Staff/Nursing/Pharmacy Committee Meetings on a monthly basis (i.e. Unit Based Committee, Therapeutic Standards Committee, Drug Use Evaluation Subcommittee, ADR/Medication Error Subcommittee, RCA/FMEA Ad hoc Committee).
Performs other related duties as assigned by authorized personnel to provide departmental flexibility to patient care needs including those for emergency situations.
* Provides help or coverage to others when needed.
* Assumes dispensing responsibilities for weekend coverage every 4 weeks.

Required Education and Experience
Required Education: Doctor of Pharmacy degree from an accredited college of pharmacy
Required Experience: PGY1 Residency
Preferred Education, Experience & Cert/Lic
Preferred Experience: PGY2 Residency
Additional Technical Requirements
* Extensive knowledge of drugs, medical therapeutics, pharmacokinetics, diagnosis and disease states.
* Ability to establish and maintain working relationships with various members of the health care teams.
* Ability to communicate clearly in oral and written form.
