Responsibilities & Requirements
The Medication Safety Clinical Pharmacist works collaboratively with the Kettering Health Medication Safety Officer. Participates in various multidisciplinary teams related to safe medication use, patient safety, policy review, and others as necessary. Continually prioritize the efficient use and maintenance of the Drug library within the Intravenous Smart Pump system. Develop and provide necessary education on a system wide level related to safe medication practices. Proactively track, trend and analyze medication safety data to identify areas for opportunities to enhance medication and patient safety. Reviews high volume, high risk, and problem prone or critical aspects of care processes for prioritization of potential medication safety improvements or initiatives. Serves as a resource to the organization on medication safety matters. Assists in the development and review of pharmacy, hospital or organization related medication use policies and procedures related to safe medication safe practices and comply with regulatory standards. Proactively engages with all pharmacy sites, pharmacy leadership, staff, and other team members to promote medication safety practices.
Job Information Continued:
* Identify and implement best practice of safe medication processes throughout the medication- use systems to improve and sustain medication safety and patient safety culture within organization
* Evaluate, categorize, and trend medication safety reports
* Investigate medication errors, adverse drug reactions, and identify system deficiencies
* Manage and update the drug library within the Smart Pump infusion system and other systems and devices that are used for drug delivery to optimize safety
* Actively contribute, and provide leadership in related committees, groups or initiatives related to medication safety
* Participate in preparation of reports intended to document the impact and quality of medication safety initiatives
* Participate in root cause analysis and lead subsequent solution efforts
* Professional engagement and communication with various disciplines and specialties
* Provide researched, accurate, and timely responses to safety concerns presented by safety, quality, pharmacy, nursing, physician, and other disciplines and specialties
* Assess compliance with regulatory standards
* Working Conditions:
Contact with patients under a wide variety of circumstances. Subjected to interruptions in work flow, requiring excellent multitasking skills. Occasional pressure due to multiple calls, drug information, or medication safety inquiries.
* Physical Demands:
Light physical effort (lift/carry up to 10 lbs). Frequent prolonged standing/walking. Occasionally lifts supplies/equipment. Manual dexterity and mobility. Occasional reaching, stooping, bending, kneeling, crouching.
Skills and Experience:
Knowledge - The clinical pharmacist should have a broad understanding of:
* Strong knowledge of medication effectiveness and impact of medications on patient care
* Health care technologies used in the medication selection process
* Pharmaceutical care

Minimum Education:
* Graduated with B.S. in Pharmacy or PharmD from an ACPE accredited School of Pharmacy
Required Qualifications:
* Licensure: Active Registered Pharmacist License in the State of Ohio
Preferred Qualifications
Preferred Experience:
* Three years inpatient hospital pharmacy experience and or completion of ASHP accredited pharmacy residency program
* Ability to develop strong interpersonal relationships
* Flexible attitude and ability to adapt to schedule and workload changes
* Professional presentation and demeanor
* Collaboration skills and effective written and oral communication skills
* Knowledge of federal, state, and local laws governing Pharmacy Practices and Pharmacy professionals
