POSITION SUMMARY: The HIV Clinical Pharmacist Specialist is responsible for providing direct pharmaceutical care, serving as a drug information and patient education resource and assisting with monitoring of human immunodeficiency Virus (HIV) and Hepatitis C Virus (HCV) outpatients enrolled in the infectious Diseases Clinic or inpatients at UMCNO.

Pharm.D., PGY 1 Residency Program

Preferred:

PGY 2 in Infectious Diseases

Board certified infectious diseases pharmacist (BCIDP) and or HIV pharmacist (AAHIVP)

Experience:

Required:

2 to 3 years' experience in general Infectious disease and 1-year experience in HIV

License/Certification:

Currently licensed or registered to practice pharmacy in Louisiana.

Board certified infectoius disease pharmacist (BCIDP) and or HIV pharmacist (AAHIVP), preferred.

Knowledge, Skills, And Abilities:

Adheres to University Medical Center New Orleans rules and regulations. Maintains standards based on accreditation bodies such as the Louisiana State Board of Pharmacy, Joint Commission, CMS, etc. Maintains state license and continuing education requirements commiserate with position. Possess level of computer skill necessary to order within an Electronic Health Record system. Basic knowledge of inventory management regarding drug shortages. Ability to prioritize tasks and be self-motivated. The incumbent must have the ability to communicate effectively with people at all levels utilizing written and verbal skills. Willingly accepts additional responsibilities as needed to help department as well as other duties as assigned.

POSITION DUTIES:

50%

Participate in provison of patient-focused services. Provide comprehensive pharmaceutical care to patients receiving antimicorbial therapy and HIV/HCV for patients in both the inaptient and outpatient settings. Evaluate antimicrobial and HIV/HCV regimens for appropriateness of use, dose, dosage form, regimen, route, therapuetic duplication, and drug interations. Discuss medication order changes/clarifications with the prescribers, document any hcanges in patient and pharmacy records, and inform others of medicationorder changes. Prodvie patient consultations as requested for pharmacokinetics, antimicorbial therapy choice, drug information...etc. Perform medication reconciliation services for patients; working with providers, case managers, and ancillary staff as a member of the multidisciplinary team in order to provide the best care for patients; participating in clinical quality management activities as assigned. Also serves as a liaison between clinic/hospital staff and the pharmacy department.

Develop, monitor and adjust antimicorbial and antiviral therapy service. Collaborate with infectious diseases physicians, pharmacy clinical managers and other healthcare professionals. While also developing interventional strategies to maximize appropriate antimicrobial and antiviral utilization and improvement in patient outcomes including but not limited to antiviral guidelines, clinical pathways, policy development and formulary management. Establish a means to report and monitor ongoing antimicrobial and anti-retroviral utilization and identified metrics showing effectiveness of ASP program. Identify opportunities for improvement regarding antimicrobial and anti-retroviral use, including but not limited to performing medication use evaluations (MUEs or QA) and conducting prospective audits. Perform formulary evaluation and class reviews for antimicrobials and anti-retrovirals and present to P&T committee. Documentation of all activities/ interventions for cost savings and outcome purposes.

Collaboration and Education of Hospital and Pharmacy department Staff: Collaborates with Hospital and Pharmacy staff to insure timely resolution of problems and questions. Manage and direct the hospital's ID PGY2 program. Fosters team-work and takes responsibility to ensure delivery of high-quality care. Provides ongoing education to hospital staff and pharmacists by developing educational material and competencies and providing in-services as needed. Awareness of regulatory standards: Demonstrates awareness and understanding of documenting information in the medical record, JCAHO standards and State Board of Pharmacy requirements. Demonstrates awareness and understanding of departmental and hospital policies, and health-system initiatives.
