Description:JOB SUMMARY

Provides a variety of pharmacy services, (inpatient and outpatient infusion), to include therapeutic drug monitoring, drug regimen review, patient and staff education, medical staff interaction, resident teaching and distribution pharmacist duties under the general direction of the Pharmacy Director or other Pharmacy Leadership.

JOB DUTIES

Facilitates the development, implementation, monitoring of adherence to, and administering departmental policies and procedures. Interprets policies and procedures to associates. Recommends and implements agreed-upon long and short-term departmental goals and objectives in order to enhance department services. These goals are aligned with corporate goals and objectives. Assists associates in the development, administration, and improvement of all pharmacy programs and services. Independent development of drug use evaluations, ADR reporting, Pharmacokinetics dosing service, and nutrition support team, development of new clinical services, pharmacy student internship rotations. Actively participates in the Pharmacy and Therapeutics or similar functioning committee, pharmacy and medical family practice residency, and educational programs to institutional associates. Performs the duties and responsibilities of the position of pharmacist as dictated by staffing situations or workload. Including but not limited to: computer order entry, preparation and legal distribution of drugs and other pharmaceuticals to patients. Reviews medication orders for clarity and correctness. Ensures appropriate drug, dose, route, and timing of needed medications. Communicates effectively, tactfully, continuously, clearly and positively with the entire scope of customers, visitors, and staff. Obtains clarification of medication orders. Supply information regarding the safe and effective use of medications to associates, physicians, patients or other groups as needed. Interviews trains and contributes to the evaluation of the performance of pharmacy associates where applicable. Involvement with associates to improve clinical quality, customer satisfaction, and internal employee satisfaction. Independently reviews and makes recommendations regarding medication therapy management. Attends rounds or patient care conferences as required. Evaluates utilization of resources and cost effectiveness of therapy decisions. Maintains current knowledge regarding pharmacy, hospital, and healthcare related topics. Develops and/or provides for in-service education and up-date programs for the pharmacy department as well as other hospital associates. Participates in the pharmacy residency program. Performs other duties consistent with purpose of job as directed.

JOB SPECIFICATIONS AND CORE COMPETENCIES

Education: Requires the knowledge of theories, principles, and concepts normally acquired through the completion of the degree of PharmD, (preferred), or B.S. in pharmacy or successful completion of the foreign pharmacy equivalency exam. Residency preferred but not required.

Licensure: Must hold a valid Indiana pharmacy license or be a graduate of an accredited school or college of pharmacy and licensed in the state of Indiana as a pharmacy intern.

Experience: Associate must have a proficiency in the practice of pharmacy with an emphasis in Pharmacacokinetic monitoring, nutritional consults, and drug information services that is usually obtained from graduation of an accredited college of pharmacy or passing of a pharmacy foreign equivalency examination and two years of hospital/institutional pharmacy practice experience or completion of a pharmacy practice residency.

Associate must demonstrate independent critical thinking skills and the ability to appropriately and effectively convey concepts to a diverse population of persons with varied educational experience.

Must demonstrate the desire and aptitude to teach pharmacy residents.
