Description
The Clinical Pharmacist duties include but not limited to: oversees the clinical aspects of the medication management; manages warfarin therapy in the anticoagulation clinic; oversees the antibiotic stewardship program; provide education to medical residents, pharmacy staff, and nurses; precepts pharmacy students; perform medication use evaluations and optimizing medication use in the facility; participates in the pharmacy and therapeutics committee; provide clinical and educational supported as needed for the institution.

Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE) with a minimum of a Bachelor's Degree. Doctor of Pharmacy degree preferred. PGY1 residency training or 2 years of hospital experience preferred; antimicrobial stewardship. Licensure in good standing as a Registered Pharmacist in the state of practice. Proficient computer skills using Pharmacy systems and Microsoft applications (i.e., Outlook, Word, Excel.). Effective verbal and written communication skills. Knowledge of patient counseling, drug therapy, and drug-drug interactions. Certifications in Immunization and Medication Therapy Management a plus.
