Position Description
Provides clinical pharmacy services including but not limited to assessment, monitoring and optimization of patient specific pharmaceutical care. Serves as a departmental and organizational resource in area(s) of expertise and provides educational programming and staff development to physicians, pharmacists, nurses and other practitioners in assigned specialty area. Serves all patient populations including neonates, pediatrics, adolescents, adults and geriatrics.
Position Requirements
REQUIRED:
* 1. Graduate of an accredited School of Pharmacy.
* 2. Doctor of Pharmacy, OR a. Master's Degree with completion of a residency in the assigned clinical area or equivalent experience.
* 3. Two (2) years previous hospital pharmacy experience.
PREFERRED:
* Certification by Board of Pharmacy Specialties (BPS) is strongly encouraged in the clinical practice specialty - Board Certified Pharmacotherapy Specialist (BCPS), Board Certified Nutrition Support Pharmacist (BCNSP), Board Certified Oncology Pharmacist (BCOP), etc.
* Oncology Experience
