JOB SUMMARY:
Specializing in Pain Management, Palliative Medicine and Opioid Stewardship. Responsibilities also include areas of formulary management, drug information, practice advancement, process development and coordination of associated EHR enhancements. The successful candidate will have opportunities to serve as a mentor and educator for pharmacy residents, pharmacy students, pharmacy staff and healthcare professionals across other disciplines.
Collaborate and consult with physicians, nurses, and other healthcare professionals to ensure safe, efficacious and cost-effective prescribing, dispensing, administration and monitoring of medication in the provision of pharmaceutical care, In doing so, the pharmacist must have current, specialized drug knowledge and demonstrate judgement, communication skills, self-direction and motivation to affect positive interventions in patient outcomes beyond those exhibited by those in a Clinical Pharmacist I position. Participate in educational programs as necessary, to provide basic and advanced services of the Pharmacy Department. Additionally, perform in the Clinical Pharmacist I role with its accountabilities and job activities pertaining to drug distribution, supervision and non-dispensing functions.
REQUIREMENTS:
Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline. Completion of a PGY-1 Pharmacy Practice Residency or a minimum of three (3) years direct patient care experience in hospital setting where knowledge is applied toward pharmacotherapy, resulting in a measurable patient outcome. 3-5 years of experience. Registered Pharmacist in Maryland, Professional membership recommended, Board Certification preferred.
