Baylor St. Luke's Medical Center is an 881-bed quaternary care academic medical center that is a joint venture between Baylor College of Medicine and CHI St. Luke's Health. **Responsibilities**

Baylor St. Luke's Medical Center is looking to add another passionate Medication Safety Clinical Pharmacist II to support our department of Pharmacy and join our amazing and already established team of Pharmacists! Our hospital system is a well-recognized system with some of the top programs in the nation! We are looking for our Pharmacists to provide exemplary patient care to meet our patient's needs. **_What you'll do:_**

+ Oversee the development and assessment of a comprehensive Pharmacy Medication Safety Program for the Department of Pharmacy that monitors and ensures the integrity and safety of services provided by the department

+ Maintain ongoing contact and serves as the Pharmacy liaison for communication and activities required for hospital accreditation

+ Assist with the development and maintenance of institutional and departmental policies related to medication use

+ Help build and implement patient safety systems related to a variety of safety initiatives, including medication processes, promoting the culture of safety for both patients and staff in all healthcare settings

+ Works with all disciplines and levels of staff within the facility, to include but not limited to, front-line staff, top-level management, and patient safety teams (e.g., RCA and FMEA teams) to reach patient safety goals

+ Will relay relevant patient safety information, lessons learned, and understanding facility issues, as well as those that have potential impact beyond the local level. Works to implement patient safety improvements initiated at the national level.

+ Assist in the provision of operational, analytical, and evaluative support regarding medication or patient safety matters. Will provide routine reports to P&T and other Quality and Patient Safety Committees regarding medication errors, audits, or other needed items.

+ Evaluates and addresses challenges of overall safety within a complex healthcare system.

+ Keeps the Clinical Pharmacy Manager and Director of Pharmacy apprised of activities, trends, observations, recommendations, etc., in support of patient safety, medication safety and healthcare system goals and objectives.

+ Provides clinical support, data analysis, performs administrative work, uses research principles, and provides education for our student and resident programs

**Qualifications**

**_Requirements:_**

+ Registered Pharmacist with the Texas State Board of Pharmacy in good standing-(RPH)

+ Aseptic Technique - Complete an ACPE approved 20 hour sterile products course within 90 days of hire

+ PGY1 Pharmacy

**_Preferred qualifications:_**

+ PGY2 in Medicaion safety

+ 5 Years preferred in medication/patient safety, conducting RCAs and FMEAs, as well as managing an adverese event reporting systm

**_Skills:_**

Clinical Pharmacist / EPIC / Sterile Compounding / Medication Safety/ Patient Safety/ Quality
