We are looking for Clinical Pharmacist for the HCS PIPS Burbank Sterile Compound Direct Department at Home Care Services.
Location: Burbank, CA
Work Schedule: Full time - 80 Biweekly Hours
Shift: 8-hour, Days
Job Summary:
Provides pharmaceutical care to home infusion patients. Coordinates care with patients, family/caregivers, physicians, nurses, and other pharmacy personnel.
Essential Functions:
* Receives, reviews, and interprets written/oral prescriptions from physicians.
* Reviews appropriateness of orders based on age, diagnosis, and other relevant patient specific criteria.
* Prepares intravenous drug admixtures using aseptic technique.
* Acquires and analyzes lab results and other clinical data and provides appropriate clinical monitoring.
* Assists in the teaching of patients and their family members/caregivers about the patient's medications, directions for use, and other information as deemed necessary. Documents teaching on the computer's EMR notes.
* Participates in weekly case conferences.
* Provides appropriate drug information to physicians and nursing staff.
* Assists in coordinating the delivery of medication and supplies to the patient's home.
* Orders infusion pumps appropriate to patient's therapy.
* Maintains and updates pharmacy charts (i.e. pharmacy plan of care, progress notes, medication record, etc.).
* Writes progress notes that document all communication with the patient, caregiver, nurse, or physician. Also documents changes in the patient's therapy or condition.
* Calls patients on a regular basis to determine their progress, as appropriate.
* Assists Pharmacy Director in maintaining all required logs and documentation for meeting regulatory standards.
* Participates in the quality assurance and performance improvement processes.
Skills:
* Knowledge of pharmaceutical chemistry, materia-medica, and pharmaceutical compounding methods.
* Knowledge of aseptic technique, ability to work efficiently with medical information resources to accurately and appropriately answer drug information questions and research medication therapy concerns.
* Knowledge of clinical pharmacy practice to include: patient monitoring, interpretation of drug serum levels, literature evaluation, formulary enforcement, and written or oral presentations to health care professionals and patients.

* Preferred: Knowledge of home infusion therapy, reimbursement plans, and computers.
* Preferred: Knowledge of JCAHO standards, state and federal regulations related to pharmacy practice.
Minimum Position Requeirements:
Education: Education to meet certification, license or registration requirement.
Experience: 2 years Experience in infusion pharmacy or IV therapy in a hospital setting.
Licenses and Certifications:
* Pharmacist Licensure in State of California.
* Registered Pharmacist in the State of California.
