POSITION TITLE: ANTIMICROBIAL STEWARDSHIP INTERNAL MEDICINE CLINICAL PHARMACIST SPECIALIST

DESCRIPTION

The antimicrobial stewardship Internal Medicine clinical pharmacist specialist provides collaborative pharmaceutical care to patients at RWJ University Hospital Rahway through clinical services, research, and teaching.

QUALIFICATIONS

Required:

+ Qualified candidates must have graduated from an ACPE-accredited school of pharmacy

+ Candidates should also be self-motivated, have good communication, organizational, and interpersonal skills, and possess or be eligible for licensure in New Jersey State.

Preferred:

+ Completion of a PGY1 Pharmacy Residency in general practice

+ Board certification in infectious diseases & or pharmacotherapy

+ Experience and leadership in infectious diseases or internal medicine preferred.

+ Experience and leadership in chronic disease management /ID and/or Internal Medicine preferred.

RESPONSIBILITIES

The major job responsibilities of this position are: pharmaceutical care to patients, pharmaceutical care to hospital patients on medical services, monitoring, clinical interventions, cost-effective care analysis, teaching, and pharmacy representation in Medical Center committees. The following activities are components of the position to assist in meeting the major job responsibilities:

+ + Provide cost-effective pharmaceutical care to patients by conducting direct patient care activities using a consistent approach that reflects the philosophy of pharmaceutical care and that is performed with the efficiency and depth of experience characteristic of an experienced antimicrobial stewardship & internal medicine pharmacist.

+ Participate in patient care rounds with hospitalists through collaborative interactions with prescribers, nursing staff, clinical pharmacists, and pharmacy students to optimize antimicrobial use and ensure compliance with evidence based practice.

+ Participates in stewardship activities set forth by the pharmacy clinical manager and director of pharmacy.

+ Manages pharmacy dosing protocols with infectious disease and P&T committees for vancomycin and other medications as needed.

+ Develop and conduct target drug programs and medication use evaluations as needed and report results to the Pharmacy and Therapeutics Committee to promote process changes to optimize medication utilization.

+ Create and maintain disease specific order sets to aid in evidence based guided care of patients

+ Monitor high risk rules in the institution s web-based surveillance tool pertaining to narrow therapeutic index drugs and antibiotics for appropriateness and follow up with medical staff to address medication management issues.

+ Manage and run antibiotic stewardship local committee and participates in system collaborative as needed.

+ Acts as a primary preceptor and mentor for pharmacy students to aid in ensuring successful completion of all rotational requirements as set forth by their perspective schools of pharmacy.

+ Provide lectures and other informative activities on clinical topics in to health-care workers.

+ Provide clinical support for quality measures through evaluation and analysis of related data.

+ Complete a minimum of one quality improvement / research project per calendar year.

+ Provide coverage of the centralized pharmacy as determined by need.
