Dispenses medications, monitors patients' drug therapies, and provides consultation to medical and nursing staff.

  • Function in collaboration with the hematology/oncology and inpatient pharmacy team, with both inpatient and outpatient responsibilities

  • Attend hematology/oncology rounds, and provide pharmacy services in the interdisciplinary practice at NCCC.

  • Collaborate with hematology/oncology providers to enhance patient-centered care.

  • Provide safe and effective drug therapy management to our patients.
  • Responsible for chemotherapy order review, monitoring, and adverse effect management

  • Monitor the care plan for regimen-specific adherence and toxicities to the patient's oral chemotherapy regimen

  • Oversee and direct the preparation of chemotherapy

  • Assist with transitions of care, supporting clinical trial protocols, staff development, and precepting of hematology/oncology students and residents.

  • Participate in order set development and collaborate with appropriate committees for implementation.

  • Develop medication-related policies, procedures and protocols, and promote safety initiatives related to the oncology patient.

  • Participate in pharmacokinetics dosing service activities, hospital committees and other duties as assigned.

  • Maintain Licensure to practice pharmacy in the State of NH.

Minimum Qualifications:
  • Graduate of an Accreditation Council for Pharmacy Education (ACPE) School of Pharmacy with a Bachelor's of Science in Pharmacy required.
  • Doctor of Pharmacy (Pharm.D.) degree preferred.
  • Completion of a post-graduate residency preferred.
  • Previous health system experience preferred.
  • Must be able to work independently with minimal direct supervision, to set priorities, make critical decisions, respond quickly to emergency requests, handle frequent interruptions, and adapt to changes in workload.
  • Good written and verbal communication skills are essential.

Required Licensure/Certification Skills:
Must be currently licensed to practice in the state of NH.
