Job Description:

Description: The Clinical Pharmacist Ambulatory - Part Time (PT) works with other clinical specialists to ensure quality multi-disciplinary patient care as well as drug therapy management. The Clinical Pharmacist performs drug utilization reviews, monitors for drug interactions, and potential adverse events while seeing patients.

Typical Duties:

* Provides clinical leadership for pharmaceutical care in area(s) of responsibility.
* Develops and optimizes pharmacotherapeutic regimens through collaborative practice agreements.
* Performs drug utilization reviews, monitors for drug interactions, and potential adverse events.
* Works to minimize polypharmacy in our patients and develops patient medication action plans with self-management goals.
* Preceptor for pharmacy students or residents, as well as participating in didactic and experiential education.
* Participates as a key component to the core multidisciplinary team involved in patient care in the Medical Home.
* Communicates and presents clinical interventions and medication action plans as part of the patient care team.
* Provides drug information, dosing, and monitoring services to practitioners including population management assistance when necessary.
* Provides educational materials and in-service for medical, nursing, and pharmacy team members.
* Assists with staff development and provides staff with access to continuing education programs.
* Maintains excellent communication within the Health Center pharmacy departments and with other Health Center team members.
* Assists in the development of Pharmacy and Therapeutics (P&T) policies, criteria, guidelines and formulary analysis.
* Assists with coordination of Joint Commission Standards and pharmacy safety initiatives.
* Performs other related duties as assigned.

Qualifications:

Required Education and Experience:
* Doctorate of Pharmacy (Pharm.D.) or equivalent from an accredited College of Pharmacy.
* 1 year residency through the American Society of Health - System Pharmacists (ASHP)
OR
* Doctorate of Pharmacy (Pharm.D.) or equivalent from an accredited College of Pharmacy.
* 4 years of direct patient care that demonstrates medication management.
Preferred Education and Experience:
* 2 years residency through the American Society of Health - System Pharmacists (ASHP)
OR
* 5 years of direct patient care that demonstrates medication management.
Required Licensure/Certification/Specialized Training:
* Pharmacy license from Texas State Board of Pharmacy or eligible for same.
* Preceptor with TX State Board of Pharmacy or complete within 3 months of hire.
* Specialty Board Certification or complete within 24 months of hire.
* Immunization Certification or complete within 6 months of hire.
* Current JPS recognized Basic Life Support (BLS) certification and must be maintained throughout employment. May complete within 3 months of hire.
* Must maintain certifications throughout employment.
