Occ Summary Work PerformedN/AKnowledge, Skills a nd AbilitiesPerform other related duties incidental to the work describe d herein.Level Characteristicssystems.records.Provide pharmaceutical car e services to the patients of the DukeUniversity Health Systemincluding, but not limited to oversight, monitoring, preparation,dispensing and pr operdocumentation of patient medication therapy as well as providing dru ginformation, patientinformation and oversight ofinformation and dispens ing systems; ensurecompliance with Federal,State and local laws and regu lations as well as proper storage anddocumentation. May alsoadvise physi cians on matters pertaining to drug usage and control;participate in the developmentand delivery of continuing education programs for Pharmacy, PatientCare Services, Medical Staffand students.Review and monitor patie nt's medication therapy and orders written byappropriate caregivers for safety and efficacy considering specificpatient conditions and healthsta tus; advisepatient care team of issues with, or possibleenhancements to, patient's medicationtherapy and document such interventions. Prepare, l abel and dispensemedications for patients inaccordance with physicians' prescriptions; ensure compliance withFederal, State and local lawsand re gulations pertaining to the dispensing of drugs and narcotics andthe mai ntenance ofrequired Maintain appropriate records of dispensing and overs ee andmaintain the systems thatcharge or credit for medications and medi cation treatments. Providedrug information and educatecaregivers, patien ts and students regarding medications and theirproper use and monitoring .Oversee and monitor all aspects of the medication process; collect data and present reports andrecommendations regarding issues with the medicat ion Ensure thatappropriate medications areavailable for patients and pro perly procured, prepared, stored andmaintained.Supervise the process and technical personnel who assist inpreparation, maintenance anddispensing of drugs and supplies.Provide support, education and oversight for inve stigationalmedications for patients; provideinformation regarding their preparation, labeling and availabilitywithin the medication system;maint ain appropriate and required records and documentation forinvestigationa l drugs.Provide specialized pharmaceutical services to improve drug usag e andtherapeutic outcomesincluding advising physicians on issues concern ing drug therapy, theinherent toxicity of drugs andside effects, as well asassisting in the prescription of appropriatedoses.Contributes to the education of students and pharmacy residents.Maintain liaison relations hipswith medical and nursing staff; provide timely information pertainin gto pharmaceutical supplies,drug usage and compatibility, state and fede ral regulations regardingdrug controls, and JointCommission standards.Co nduct and evaluate medication histories, assess compliance andsuggest mo difications so as toprescribed drugs; participate inachieve desired outc omes; instructpatients in the proper useofpatient care rounds with mul tidisciplinary teams to evaluate patientprogress. Individualizemedicatio n regimens using sound principles, accounting forpharmacodynamic andphar macokinetic variations in drug absorption, distribution, metabolismand e limination withresponsibility for establishing and continually improving the deliveryof pharmaceutical care tothe department with the same.patie nts within areas of direct responsibility and assisting others inConduct and participate in research including, but not limited to,performing as the primaryinvestigator or co-investigator for research programs which will impacton the delivery of qualitycare or examine the pharmacoeconomi c impact of providing care to thepatients inthe DukeHealth System.Abili ty to communicate clearly in oral and written form.Extensive knowledge o f drug products, medical therapeutics,pharmacokinetics,and diseasestate s. Minimum Qualifications

Education

Doctor of Pharmacy Degree OR BS Pharmacy Degree

Experience

With a BS Pharmacy degree, two years of pharmacy practice experience, or acceptable board certification, or completion of an accredited residency program and demonstrated ability to achieve positive patient outcomes in a similar environment is required. With Doctor of Pharmacy degree, no experience is required.
