Reporting to the Specialty Clinical Pharmacy Manager, this position is to provide the services of a licensed pharmacist which includes the provision of specialized clinical and/or MTM services to patients in the ambulatory setting to patients who range in age from neonates, infants less than one year of age, children and adolescents ages 10-18, adults ages 19-65, and the elderly over age 65.

This position is to function at higher decision making levels regarding drug therapy, proactively affecting drug use by making recommendations at the time prescribing decisions are being made or, in some cases, making independent decisions about drug therapy in cooperation with other health team members. This position is responsible for the direct clinical supervision of patients, and ensuring compliance with all legal, regulatory and institutional directives. This pharmacist will interact directly with patients, prescribers, nurses, and care coordinators to develop an interdisciplinary approach to patient care. Additionally, the pharmacist will be responsible for the development and implementation of additional Specialty Pharmacy clinical opportunities and will be expected to contribute to the financial viability of the program, department and institution.

+ Pharm.D. or BS in Pharmacy with equivalent experience

+ Licensure in the State of Oregon

+ Ambulatory pharmacy practice experience

+ Demonstrates the ability to:

+ Enforce pharmaceutical care principles safely and accurately

+ Make age-related modification of drug therapy

+ Make disease-related modification of drug therapy

+ Evaluate medical literature

+ Work independently without minimal direct supervision

+ Handle multiple tasks and frequent interruptions

+ Communicate effectively both formally and informally, including presentations

+ Project manage

+ Work with a team professionally and collaboratively

+ Set priorities to respond quickly to emergent situations/requests

+ Work cooperatively with health-system and pharmacy staff

+ Communicate effectively and professionally (orally and in writing)

+ Perform work accurately with attention to detail

+ Exercise sound professional judgement

+ Perform systematic problem solving

+ Enforce practice guidelines, standards, and medication related policies

+ Make critical decisions

+ Perform key computer related skills including use of the EMR, common word processing, spreadsheet, medical literature and medication databases

+ Possesses excellent knowledge of third party payors and insurance billing guidelines of Specialty Medications Possesses computer skills including use of MS-Word, Excel and Access databases.

+ Completes initial and continued competencies as required for maintenance of standard of practice, legal regulatory compliance and OHSU policies

+ Ability to handle competing demands on their time from clinic staff and/or patients and will need to effectively multi-task and prioritize demands

+ Must deal with intense situations on a daily basis

+ Ability to provide clients with excellent customer service

+ Ability to handle frequent interruptions (work is often dictated by external factors) that may lead to mental fatigue or stress

+ Standing, walking, stooping, reaching, talking and hearing

+ Clarity of vision at 20 feet and 20 inches or less; adjustment of eye to bring object into sharp focus

+ Ability to handle 1 - 5 pounds by hand frequently; telephone, pager, copier, fax, scanner, personal computer/terminal, common application software

+ Must be able to perform the essential functions of the position with or without accommodation

+ Advanced Pharmacy Practice Residency OR 5 years of equivalent experience

+ Additional Certification (BCPS, BCACP, BCPPS, BCIDP, etc.)

+ Experience in clinical program development

+ Direct patient care or medication therapy management experience

+ State of Oregon Preceptor License.
