We are seeking a Staff Clinical Pharmacist who will be responsible for providing and coordinating pharmaceutical care services and collaborative drug therapy management for patients. The Pharmacist will serve as a resource to the patients, physicians, pharmacists, nurses and other members of the health care team. Provide prescribed medications, drugs, and other pharmaceuticals, compounds medications, and prepares special intravenous solutions according to professional standards and practices. Assure safe and effective medication use and assures that drug storage is safe, controlled, and meets required conditions. Demonstrate proficiency in delivering care to assigned age specific patient population (e.g., infants, adolescents, adults, geriatrics, pediatrics), and participate in related continuing education.

In this position you will:

* Implement and maintain established department, unit, and hospital policies, procedures, and objectives as well as improving organization performance program, safety, environmental, and infection control policies
* Demonstrate excellent customer service and positive interpersonal relationships with physicians, staff, patients/families
* Actively participate in daily patient care to identify patients for therapeutic monitoring and to discuss potential interventions
* Participate in ongoing quality assessments and improvements of clinical pharmacy activities in collaboration with physicians and implements necessary action as appropriate
* Provide comprehensive pharmacy services to patients and may include drug therapy assessment based upon evaluation and interpretation of patient history, age, medical condition, surgical history and objective physical biochemical data; development of therapeutic goals and endpoints of drug regimen; preparation of discharge orders and patient education within scope of practice
* Effectively communicate verbally and/or in writing pharmacy functions to the interdisciplinary team
* Serve as a drug information resource person to physicians, patients and other health care staff
* Supervise employees directly to ensure safe, legal, and effective drug distribution and use
* Monitor drug therapy and suggest alternate drug therapy when appropriate
* Participate in QA review; regularly document inspection of all medication storage areas
* Check stock on a regular basis to identify and reorder outdated stock and ensure that stock is maintained in accordance with manufacturer requirements
* Ensure secure storage of narcotics and other controlled substances on a regular basis and verifies inventory control
* Prepare clear and concise clinical briefs for use by nurses, medical staff and pharmacy
* Interpret and clarify orders for all types of medications
* Act as a liaison and consult with nursing and medical staff concerning prescribing, transcribing, dispensing, administration/monitoring of medications; report patient adverse drug reactions, medication variances and complaints; participate in educational activities and programs for the interdisciplinary team
* Ensure delivery of quality pharmacy services for inpatients, outpatients, and employees in accordance with established legal requirements
* Compound, label, and package medications and pharmaceuticals meeting professional standards and guidelines
* Maintain and enhance professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops; demonstrate a comprehensive clinical pharmacy knowledge base
* Attend meetings and participate on committees as directed/required

Required qualifications:

* Bachelor's Degree
* Current California Licensed Pharmacist
* Registered by the California State Board of Pharmacy as a Licensed Pharmacist
