POPULATION SERVED The position involves direct and indirect patient care for a population of patients between the ages of 18 and older. Age specific experience and/or special training and/or expertise is required to serve this population. POSITION SUMMARY Under the general supervision of the Director of Pharmacy, the Clinical Pharmacist Antibiotic Stewardship Pharmacist serves as both a supervisor and a clinical pharmacy specialist. The incumbent will also act in the absence of the Pharmacy Director and assist the Director in planning, coordinating and implementing pharmacy programs, projects, and services. He/she will be responsible for implementing and monitoring strategies to optimize the treatment of infections and reduce adverse events associated with antibiotic use. POSITION QUALIFICATIONS * This position requires graduation from an accredited school of Pharmacy and State Licensure with minimum of a Bachelor's degree. * CA pharmacy license in good standing. * Pharm D or advance degree preferred. * Must exercise sound professional judgment using initiative consistent with a high level of resident care. * Ability to work efficiently and under broad guidelines, and must frequently exercise his/her own judgment in decision making. * Regularly works with confidential information. Requires endurance to perform tasks over long periods of work hours. DUTIES RESPONSIBLITIES * Provide regular updates on antibiotic prescribing, antibiotic resistance, and infectious disease management. * Track clinical outcomes that measure the impact of interventions to improve antibiotic use. * Perform periodic assessments of the use of antibiotics or the treatment of infections to determine the quality of antibiotic use. * Participates in Infection Control Committee. * Develop and implement facility specific policies and interventions to improve antibiotic use. * Works with Pharmacy Director to supervise the work of subordinate pharmacist and technical staff; schedules, assigns, directs, and monitors the quality and quantity of clinical work performed. * Monitors the use and administration of patients' drug regimens and the appropriateness of serum drug levels; verifies dose appropriateness based on clinical goals and expectations; provides a baseline for adjustments; consults with physicians and nurses regarding the appropriateness of proposed drug use. * Collaborates with Pharmacy Director to monitor and evaluate adverse drug reactions and interventions; compiles and reports statistical information; develops quality assurance plans to assess the appropriate use of drugs. * Coordinates clinical pharmacy activities with physician, nursing, laboratory, and other personnel; develops and assists in the implementation of clinical pathways and therapeutic guidelines. * Teaches and instructs pharmacists, nurses, physicians, and others regarding the technical skills used in monitoring drug therapy; provides advice regarding the actions to undertake in solving drug therapy problems. * Coordinates pharmacy record keeping, auditing, and other drug control policies, procedures and activities a required by County, State, and Federal law; communicates with insurance companies and health benefit representatives regarding reimbursement and formulary matters. * May interpret, fill, and dispense prescriptions and perform other professional pharmacist duties as assigned. * Serves as a resource for the professional staff and answers technical pharmaceutical questions; develops protocols to monitor drug therapy. * May advise physicians and other health practitioners on the selection, dosage, interactions and side effects of medications. * Establishing or maintaining patient and/or customer profiles. * Program, direct, review and rectify pharmacy procedures. * Performs other related duties as assigned.
