Overview

Atlantic Health System is seeking a full-time Clinical Pharmacist for the Pharmacy Department at Morristown Medical Center to work from day shift.

Responsibilities

The Transitions of Care clinical pharmacist will create and oversee ongoing initiatives, including optimization of the medication reconciliation process, discharge counseling, and patient education with a focus in patients with cardiac conditions such as heart failure and myocardial infarctions. The clinical pharmacist will participate in P&T and patient education committees and will serve as a preceptor and mentor for pharmacists, residents and students. The Transitions of Care clinical pharmacist will work with teaching and nonteaching medical teams. Medication utilization evaluation and research opportunities are also available.

Additional responsibilities include: • Complete data collection of outcomes and present metrics to support ongoing initiatives• Provide education for inpatients in need of counseling, as identified by nurses and practitioners• Provide comprehensive pharmaceutical care to our patients including: clinical intervention; teaching and pharmacy representation on various committees; participation in daily patient care rounds; medication order clarifications with the prescriber, document any changes in patient and pharmacy records and inform others of medication order changes; monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions, provide pharmacokinetic consultations as required; prevent, detect, monitor, document, and report adverse drug reactions and medications errors; participate in the drug therapy management of medical emergencies, promote the use of the formulary by converting nonformulary orders to formulary when possible

Qualifications

EDUCATION/QUALIFICATIONS:

Must be a graduate of an accredited school of Pharmacy with BS and/ or Pharm D degree required. Completion of a PGY 1 or clinical experience required. Completion of PGY 2 preferred. Board Certification and/or ASHP Accredited Residency preferred. Current NJ Board of Pharmacy license required. Minimum of 1 year experience in a hospital practice setting, with transitions of care experience required. Experience in research activities also preferred
