Position Summary

Provides education about rehabilitation services to physicians, social workers, discharge planners, insurance companies, case managers and rehabilitation case managers to expedite patient referrals. Assures safe, appropriate, efficient and effective admission of patients into the rehab unit by performing clinical assessments of patients referred to the rehab unit. Assures completion of the intake assessment of inpatient unit.

Minimum Education Requirements

  • Associates Degree in Nursing (ASN) or Bachelors Degree in Physical Therapy, or Bachelors Degree in Occupational

  • Therapy or Masters Degree in Speech Therapy with Certificate of Clinical Competence

  • Bachelors Degree in Nursing (BSN), preferred Masters/Doctorate Degree in Physical Therapy, preferred Masters Degree in Occupational Therapy, preferred

Minimum Experience Requirements

  • 2 years of rehabilitation or other appropriate post-acute environment experience

  • 1-3 years clinical bedside experience

  • 1 year of proven marketing experience demonstrating a consistent ability to achieve objectives, preferred

  • Experience/knowledge in community health setting/services, preferred

  • Experience reviewing medical records

  • Good oral and written communication


  • Current Colorado RN license/ Current RN license from a participating state in National Licensure Compact or Current Colorado state licensure in Physical Therapy, or Current Colorado state licensure in Occupational Therapy and registered Speech Language Pathologist or Certificate in Competence from the American Speech Language Hearing Association

  • Current Basic Life Support (BLS) from the American Heart Association

  • Active Drivers License

  • Certified in Rehabilitation (CRRN), preferred

Position Duties (Essential Functions)


  • Assesses patients' needs; identifies rehab candidates and initiates appropriate communications with director and medical director to assure uniform application of admission procedures and criteria.

  • These functions are carried out according to regulatory guidelines.

  • Utilizes the appropriate leadership skills in delegating, organizing and educating coworkers and staff.

  • Coordinates and supervises appropriate levels of staff.

  • Participates in and contributes to the quality improvement process for the department and organization.

  • Demonstrate advanced clinical and communication skills contributing to the development and growth of the Rehabilitation program.

  • Work in conjunction with the Manager and Director of the Porter Center for Rehabilitation to facilitate communication between the Medical

  • Director, physicians, nurse managers, and clinicians to assure excellence in clinical services.

  • Collaborate with physicians, Rehabilitation staff, business development staff, and other clinical members to assist with the implementation and marketing of the

  • comprehensive Rehabilitation programs/services.

  • Support growth initiatives of the Rehabilitation program and actively participates in strategy and tactics. Reaches out to stakeholders to enhance the referral processes to facilitate growth.

  • Assist physicians in developing and revising clinical practice guidelines for Rehabilitation care.

  • Incorporates appropriate research finding into clinical practice: evidence based practice.


  • Verifies patient benefits and obtains authorization for reimbursement prior to admission to rehab unit when appropriate.

  • Develops educational plan by researching referral area and identifying needs and servicing those needs to increase understanding and appropriate use of the rehabilitation unit and services.

  • Demonstrate the knowledge, skill and finesse to promote the Porter Center for Rehabilitation to outside entities, such as, case managers, discharge planners, and potential new admissions.

  • Facilitate transfers into the Rehabilitation unit within established service standards and insurance guidelines.


  • Provides patient care services to patients of ages indicated including: Facilitates admission by responding to referral requests in a timely manner; maintaining effective communication with referral source and providing complete and accurate patient assessment information to the assessment team.

  • Personally develops referral sources by calling on referral agencies and provides, identifying new referral sources, conducting in-services and arranging and conducting tours. Maintains positive relationships with referral sources by performing account needs assessment on a regular basis to identify problems and working with referral source to jointly handle complaints, problems and concerns.

  • Keeps management informed by submitting required reports and results and by communicating as needed programs and needs.

  • Maintains professional and marketing knowledge by attending workshops, reviewing professional publications, establishing personal networks, participating in professional societies.


  • Develop an outreach program designed to improve growth in the designated rehabilitation unit.

  • Identify and provide education to patient/families about current and potential rehabilitation issues.

  • Provide formal and informal education to other members of the health care team to achieve desired outcomes.

  • Coordinate and participate in community education programs, health fairs and local events.


  • Maintain ongoing analysis of the program in conjunction with the Medical Director and Director of the Porter Center for Rehabilitation, providing outcome measures to assess efficacy and performance improvement of the program.

  • Maintain current database.

  • Organize, implement and attend the multidisciplinary Rehabilitation leadership councils and be able to document the treatment plans developed for patients.

  • Participate in determining goals/objectives/strategic plan for the Porter Center for Rehabilitation.

  • Participate in professional organizations, attends professional workshops/conferences.

  • Actively involved in maintaining state of the art Rehabilitation Care.

  • Assists with compliance audits, as requested.

  • Light Work - exert/lift up to 20 lbs force occasionally, and/or up to 10 lbs frequently)
