Position Summary

Accountable for targeted clinical practice engagement and performance transformation in select Colorado Health Neighborhood (CHN) physician offices. Lead engagement of both providers, staff, and practice leadership in key performance functions such as quality, efficiency, and operational standards, including but not limited to access and service. Ability to assess systematically practice performance strengths, weaknesses, opportunities, and threats. Partners with practices to evaluate performance of clinical processes and outcomes, risk management, customer satisfaction, and financial impact of clinical quality decisions. Promotes practice’s strategies to optimize reimbursement for quality improvement initiatives. Ability to evaluate based on data and assessment of

team dynamics, develop and propose improvement workplans, guide solutions, lead change management activities, partner with clinical staff to implement practice operation changes, ensure changes are sustainable, and have ability to replicate best practices in multiple locations in a rapid cycle fashion.

Minimum Education Requirements

Baccalaureate degree in Nursing, Health Sciences or Business required

Master’s degree Health Sciences, Business, or Nursing preferred.

Minimum Experience Requirements

5 years health care experience in direct practice operations, practice office leadership, or care delivery operations (primary, specialty or acute care) and quality improvement sciences or Lean methodology.

Experience in change management initiatives, tactics, project management

Working knowledge of PCMH Joint Principles and requirements for NCQA PCMH recognition Patient-Centered Medical Home (PCMH) experience, preferred

Experience with an electronic health record, required

Clinical experience, preferred Proficient use of Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) Team-building/facilitation of committees

Presentation and Influencing Experience


Certification in performance improvement methodology such as Six Sigma or LEAN recommended.

Position Duties (essential functions denoted with an * )

■Accountable for practice transformation/performance improvement results in select CHN physician offices.*

■Lead practice engagement and change management activities which will include assessment of practice performance, performance workplan development, and being onsite/available as an embedded member of the care team until performance improves.*

■Maintain a specific focus on quality, efficiency, operations, and population health standards and measures.*

■Accountable for early business development due diligence for CHN practices.*

■Quickly develop effective relationships such that role is viewed as a critical member of the medical office operations team.*

■Collaborate with other teams/resources such as CHN team members and physician leaders who set foundational expectations to CHN practices for performance standards. *

■Collaborate with FullWell suite of services and programs including but not limited to, quality, coding, care coordination, and utilization management teams to approach practices in a coordinated and strategic manner with the provider as the center of focus.*

■Lead project management and workplan lead/lag measures dashboards and document best practice approach in performance improvement to be replicated at other sites.*

■Participate in strategic review and resource alignment based on performance data.*

■Provides one-on-one direct provider performance feedback and works with practice teams to implement continuous quality improvement processes and organizational change.*

■Effectively communicate the CHN/FullWell value proposition to key stakeholders and articulate specifically howrole fits into the broader spectrum of value based care.

■Maintains competency on various information systems, including but not limited to: electronic health record, population management software, educational partner software, SharePoint collaboration software, patient experience reporting software, office software suite.*
