The Oncology Clinical Specialist provides comprehensive pharmaceutical care to oncology patients through integrated drug distribution, clinical services, research and teaching.

1. Provide comprehensive pharmaceutical care to hematology/oncology patients.
a. Participate in patient care rounds.
b. Discuss medication order clarifications with the prescriber, document any changes in patient medical and pharmacy records, and inform others of medication order changes.
c. Provide pharmacokinetic consultations as required. Monitor drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.
d. Prevent, detect, monitor, document, and report adverse drug reactions and medications errors.
e. Participate in the drug therapy management of medical emergencies.
f. Promote the use of the formulary by converting non-formulary orders to formulary when possible, but coordinates procurement of non-formulary drugs when necessary.
g. Maintain and update patient profiles with demographics, diagnosis, allergies, and current medications.
h. Reconcile pharmacy and nursing medication records.
i. Perform medication reconciliation for patients' home medication as appropriate.
j. Counsel patients on the first cycle of chemotherapy regimens.
k. Counsel patients on medications as necessary.
l. Facilitate placement/discharge planning and smooth transition to alternate site of care.
m. Assess insurance drug coverage and assist in preauthorization for medications as applicable.
n. Work collaboratively with the clinic to ensure appropriate practice.

2. Promote and demonstrate rational, cost-effective drug therapy.
. Identify and implement drug therapy where positive pharmacoeconomic impact can be achieved. The ultimate goal is to develop and assure cost effective clinical pathways and order sets.
a. Develop and conduct target drug programs and medication use evaluations as needed and report results to the Pharmacy and Therapeutics Committee.
b. Document clinical consultations and cost avoidance as part of the departmental documentation program.
c. Monitor use of expensive medications to assure use consistent with approved criteria (e.g., biotechnology drugs, chemotherapy, antibiotics, colony stimulating factors, etc).

3. Facilitate investigational drug studies.
. Coordinate the pharmaceutical care of ongoing research.
a. Provide in-service education for investigational studies to the nurses and pharmacy staff.

4. Provides education and training to members of Oncology and Pharmacy departments.
. Precept Pharm.D. students. Precept and mentor pharmacy PGY1 and PGY2 residents.
a. Provide staff development support to nurses, and physicians.
b. Facilitate personal development in hematology/oncology for the clinical staff pharmacists.
c. Assure compliance to standards and educate pharmacy staff on ASHP standards (including USP 797) for handling hazardous drugs, sterile products and preventing medication errors with antineoplastic drugs.
d. Review/update pharmacy policies related to chemotherapy.
e. Participate in Seacoast Cancer Center committees where pharmacy representation is essential.
f. Participate actively in hematology/oncology team meetings and educational teaching conferences.

Qualifications
Experience Minimum Required
o 1+ years of Clinical Pharmacy experience

Experience Preferred/Desired
o 3+ years Oncology Pharmacy experience

Education Minimum Required
o Must possess PharmD or equivalent

Education Preferred/Desired
o PGY1/PGY2 or other equivalent experience

Training Minimum Required
o Board Certified in Oncology (BCOP) within 3 yrs of hire

Special Skills Minimum Required
o Maintains competency in unique aspects of drug therapy for Hematology/Oncology patients, including factors related to age, organ function and disease states.

Licensure and/or Certifications Required
o NH Pharmacy License
