Responsibilities

A Medication Reconciliation Pharmacy Technician will be independently obtaining medication and allergy histories for patients that will be admitted to the hospital from the Emergency Department. Information will be obtained by interviewing the patient, patient family, calling local pharmacies and also using other available resources. The information obtained will be entered into a computer system and it will be used by physicians and other health care providers to determine treatment plans and medication orders during and post discharge from the hospital. In addition, Pharmacy Technician will assists the pharmacist in his/her professional responsibilities in providing pharmacy services. Assistance of pharmacists in providing pharmaceutical care to both in-patients and out-patients. Working under the direction of a licensed pharmacist, participates in the preparation, dispensing, distribution and return of medications including intravenous admixtures processing of patient drug orders, sterile and non-sterile preparations of liquids and solid dosage forms, supplies and maintains automated technology equipment, packages unit of use dosages, maintains and transports pharmaceuticals, care and ordering of supplies and equipment, training of trainees and students, and performs tasks associated with regulatory compliance, restocking of pyxis machines, as needed independently obtaining medication and allergy histories for patients that will be admitted to the hospital. Documents activities as indicated and performs other duties as required and assigned.

Qualifications

EDUCATION REQUIREMENTS- High School Diploma or GED

EXPERIENCE- Previous experience preferred.

CERTIFICATION AND LICENSURE REQUIREMENTS- National Pharmacy Technician Certification (CPhT) through the Pharmacy Technician Certification Board (PTCB) Required upon hire or within the first 6 months of employment
